经阴道骶棘韧带悬吊固定术治疗全子宫切除术后阴道穹窿脱垂15例临床分析

摘　　要：目的探讨经阴道骶棘韧带悬吊固定术治疗全子宫切除术后阴道穹窿脱垂的临床疗效。方法对西安市第四医院妇科2009年2月至2012年3月收治的15例全子宫切除术后阴道穹窿脱垂的患者进行了经阴道骶棘韧带悬吊固定术,分析其临床疗效,并进行每3个月一次的随访观察。结果所有患者进行该术式治疗均成功,无一例发生术中、术后出血或脏器损伤。15例患者术后均恢复良好,术后无一例复发,术后平均随访10个月,主观治愈率(术后患者无任何自觉症状)和客观治愈率[盆腔器管脱垂定量分期法(pelvic organ prolapse quantitation,POP-Q)分期为0期]均为100%。结论对全子宫切除术后阴道穹窿脱垂患者施行经阴道骶棘韧带悬吊固定术矫治阴道穹窿脱垂是一种安全、简单、有效的微创手术。Objective To explore the clinical effect of transvaginal sacrospinous ligament fixation in the treatment of vaginal vault prolapsed after complete hysterectomy. Methods About 15 cases with vaginal vault prolapse after complete hysterectomy were underwent transvaginal sacrospinous ligament fixation from February 2009 to March 2012. The effect of operation was analyzed and all the cases were followed up for at least three months. Results All operation were completed successfully without any complications, such as hemorrhage and neighbor organ injury during and after the operation. All cases recovered well after the operation. After ten months of follow up, the subjective and objective cure rates were all 100%. Conclusion Transvaginal sacrospinous ligament fixation is a safe, simple, effective and minimally invasive operation for patients with vaginal vault prolapsed after complete hysterectomy.
