成人股骨头缺血坏死影像诊断与介入治疗探讨(附80例报告)

摘　　要：目的 :探讨成人股骨头缺血坏死的不同影像分期的介入疗效。方法 :回顾分析了 80例病人的影像学各期表现 ,其中 ,综合影像分期Ⅰ～Ⅱ期 5 1例 ,Ⅲ～Ⅳ期 2 9例进行介入治疗。结果 :通过 12～ 3 6个月的随访观察 ,疼痛缓解率Ⅰ～Ⅱ期为 94% ,Ⅲ～Ⅳ期为 62 % ;关节活动度均有不程度增加 ;DSA检查相同时相对比 ,血管增加Ⅰ～Ⅱ期为 86% ,Ⅲ～Ⅳ期为 45 % ;骨质明显修复增生 ,Ⅰ～Ⅱ期为 5 1% ,Ⅲ～Ⅳ期为 10 %。结论 :Ⅰ～Ⅱ期介入治疗效果最佳 ,Ⅲ～Ⅳ期效果较差。Objective:To evaluate the interventional therapy on adult avascular necrosis of the femoral head (ANFH) with different imaging stages.Methods:80 patients with ANFH were treated with interventional therapy,including 51 cases with stage Ⅰ～Ⅱ and 29 cases with stage Ⅲ～Ⅳ according to image staging.Results:The follow up period extended from 1～3 years.The rate of relief of the pain was 94% of stage Ⅰ～Ⅱ and 62% of stage Ⅲ～Ⅳ,respectively.The joint function improved in all cases.In comparison with those before treatment,the rate of increase of vasculature was 86% in stage Ⅰ～Ⅱ and 45% in stage Ⅲ～Ⅳ,respectively;obvious changes of bone reparation and hyperplasia occurred 51% of stage Ⅰ～Ⅱ,and 10% of stage Ⅲ～Ⅳ.Conclusion:Patients with ANFH in stage Ⅰ～Ⅱ can get better effect than that of stage Ⅲ～Ⅳ by the interventional therapy.
