腹腔镜术后联合醋酸曲普瑞林治疗卵巢子宫内膜异位囊肿疗效观察

摘　　要：目的:探讨腹腔镜术后联合醋酸曲普瑞林治疗卵巢子宫内膜异位囊肿的临床疗效。方法:72例卵巢子宫内膜异位囊肿患者按自愿原则分为2组。对照组30例单纯行腹腔镜手术;观察组42例行腹腔镜术后联用醋酸曲普瑞林3.75 mg肌内注射,每28天1次,连用3个月。所有患者随访24个月,观察疗效、术后复发率和妊娠率。结果:观察组与对照组的有效率分别为78.6%(33/42)和43.3%(13/30),复发率分别为11.9%(5/42)和33.3%(10/30),自然妊娠率分别为46.7%(14/30)和18.2%(4/22),两组差异均有统计学意义(P<0.05)。结论:腹腔镜术后联合醋酸曲普瑞林治疗卵巢子宫内膜异位囊肿可以提高疗效,减少复发率,提高自然妊娠率。Objective： To evaluate the efficacy of triptorelin in the treatment of ovarian endometriotic cyst with laparoscopic surger- y. Method： Totally 72 patients suffered from ovatian endometriotic cyst were divided into the control group and the observation group according to the principle of voluntariness. The 42 cases in the observation group were treated postoperatively with triptorelin（3.75 mg intramuscular injection once every 28 days） for three times, and the 30 cases in the control group were received laparoscopic operation only. Result： Compared with that of the control group, the effective rate of the observation group was significantly increased and the re- currence rate was significantly declined [ the total effective rate： the observation group 78.6% （33/42）, the control group 43.3% （ 13/30）, P =0.002; the recurrence rate： the observation group 11.9% （5/42）, the control group 33.3% （10/30）, P =0.027 ,P 〈 0.05 ]. Conclusion： Triptorelin acetate combined with laparoscopic surgery in the treatment of ovarian endometriotie cyst can improve the curative effect, reduce the recurrence rate and increase the natural pregnancy rate.
