老年脑梗死患者下呼吸道感染的影响因素分析

摘　　要：目的探讨脑梗死患者下呼吸道感染的影响因素,旨在为针对性的采取预防对策提供参考依据。方法采用自编问卷收集2010年8月-2012年8月神经内科收治的128例脑梗死患者临床资料,根据脑梗死患者有无出现下呼吸道感染分为A组和B组,运用描述性统计方法和logistic回归分析法,找出脑梗死患者下呼吸道感染的影响因素并提出针对性的预防措施。结果脑梗死患者下呼吸道感染发生率为13.28%;多因素logistic回归分析发现,患者有意识障碍(OR=9.590)、吞咽困难(OR=7.253)、咳嗽无力(OR=5.046)和侵入性操作(OR=8.604)是脑梗死患者下呼吸道感染的影响因素。结论脑梗死患者下呼吸道感染发生率较高,其发生受到很多因素的影响,需要采用针对性的干预措施,减少脑梗死患者下呼吸道感染的发生。OBJECTIVE To investigate influencing factors and prevention countermeasures for elderly cerebral infarction patients merging lower respiratory tract infections in order to provide reference basis for taking preventive measures.METHODS The clinical data of one hundred and twenty-eight elderly cerebral infarction patients who were treated in the neurology department of the hospital from Aug 2010to Aug 2012were collected by the self-made general data questionnaire,and then they were analyzed by descriptive statistics and binary classification logistic regression analysis of the SPSS16.0so as to find out the influencing factors and put forward prevention countermeasures for elderly cerebral infarction patients merging lower respiratory tract infections.RESULTS The incidence rate of the lower respiratory tract infections was 13.28%in the elderly cerebral infarction patients,and the multivariate logistic regression analysis showed that the conscious disorder（OR=9.590）,dysphagia（OR= 7.253）,cough powerlessness（OR=5.046）,and invasive operation（OR=8.604）were the risk factors for the lower respirator tract infections in the elderly patients with cerebral infarction.CONCLUSION The incidence rate of the lower respiratory tract infections is high in the elderly cerebral infarction patients,there are a variety of risk factors for the infections,and we should take targeted intervention measures so as to reduce the incidence of lower respiratory tract infections.
