封闭式负压引流装置在髋关节置换术后早期感染的应用研究

摘　　要：目的观察封闭式负压引流装置在髋关节置换术后早期感染的临床疗效,为其治疗提供经验参考。方法将医院骨科行髋关节置换术并发生术后早期感染的42例患者,随机分为观察组和对照组,在抗感染治疗基础上,观察组予80kPa负压引流,对照组予20kPa负压引流,观察两组患者的治疗效果。结果观察组患者引流术第3天C反应蛋白(CRP)为(8.21±4.92)mg/L,明显低于对照组的(14.56±5.63)mg/L;观察组患者12h内引流量为(371.42±51.32)ml,总引流量为(524.51±59.51)ml,引流持续时间为(3.32±1.02)d;观察组患者12h内引流量明显高于对照组,总引流量明显低于对照组,引流持续时间明显短于对照组;观察组平均感染控制时间为(5.63±2.96)d,明显短于对照组的(9.28±4.23)d,差异均有统计学意义(P<0.05)。结论封闭式高负压引流术治疗髋关节置换术后早期感染时,引流第3天CRP及体温降低、12h内引流量多、总引流量少、感染控制时间短,有利于患者感染的早日康复,可长时间的保留置换髋关节。OBJECTIVE To observe the therapeutic effect of closed negative pressure drainage device on the treatment of early infections after hip arthroplasty so as to provide clinical experience for its treatment.METHODS A total of 42patients with early infection after hip arthroplasty were selected in our hospital,they were randomly divided into the observation group and the control group with 21patients in each group.Based on the anti infection treatment,the patients in the observation group received the treatment of 80kPa negative pressure drainage,while the patients in the control group received the treatment of 20kPa negative pressure drainage,the therapeutic effect of both groups was compared.RESULTS The concentration of C-reactive protein（CRP）on the third day of the patients in the observation group was（8.21±4.92）mg/L,it was significantly lower than that of the control group（14.56±5.63）mg/L;the 12hinternal drainage,total drainage and drainage duration of the observation group were（371.42±51.32）ml,（524.51±59.51）ml,and（3.32±1.02）days respectively,the total drainage was significantly lower than that in the control group,the drainage duration was significantly shorter than that in the control group;the average time of the infection control of the observation group was（5.63± 2.96）days,it was significantly shorter than that of the control group（9.28±4.23）days,the differences were statistically significant（P0.05）.CONCLUSION The concentration of CRP,temperature,12hinternal drainage,total drainage and drainage duration are lower or shorter when closed high negative pressure drainage is used in the treatment of early infections after hip replacement,which may contribute to the early rehabilitation of the patients and to the prolonged retention of hip replacement.
