针刺干预甲基苯丙胺戒断者记忆障碍的ERP观察

摘　　要：目的研究针刺干预甲基苯丙胺戒断者记忆障碍的中枢机制。方法以改善认知障碍为主要治疗策略,利用NuAmps 40导事件相关电位系统,从不同记忆负荷水平记录并比较甲基苯丙胺戒断者对3类汉字(包括毒品主要相关词、毒品次要相关词、无关词)的反应,分析甲基苯丙胺戒断者针刺治疗前后的工作记忆状态与ERP改变的对应关系。结果甲基苯丙胺戒断者针刺治疗后1-back的P3波幅显著高于2-back,P<0.05,1-back条件下甲基苯丙胺戒断者针刺治疗前后P3峰值有显著组间差异,P<0.05。2-back条件下针刺治疗前后无显著性差异。简单分析表明顶区(PZ电极)P3峰值有显著组间差异;1-back条件下甲基苯丙胺戒断者针刺治疗前后P3峰值有显著组间差异,P<0.05;2-back条件下甲基苯丙胺戒断者治疗前后P3峰值无显著组间差异,差异波的N450成分主要分布于顶区位置,且分布在脑区范围较大。结论 PZ电极是毒品相关n-back任务中P3成分较为明显的电极,针刺可能通过作用于此记忆位点对甲基苯丙胺所致记忆障碍产生治疗作用。Objective To investigate the central mechanism of acupuncture intervention in dysmnesia in abstinent methamphetamine abusers and to consider improvement in cognitive disorder as the main strategy of treatment. Method NuAmps （a 40-channel event-related potential system） was used to record and compare the responses of persons withdrawing from methamphetamine to three kinds of Chinese characters （including main drug-related words, secondary drug-related words and unrelated words） from different memory loads and to analyze the corresponding relationship between working memory status and ERP change in abstinent methamphetamine abusers before and after acupuncture treatment. Result P3 amplitude was significantly higher in the 1-back load condition than in the 2-back load condition in the abstinent methamphetamine abusers after acupuncture treatment （P〈0.05）. There was a statistically significant difference in P3 peak value in the 1-back load condition （P〈0.05） and no significant difference in the 2-back load condition between before and after acupuncture treatment in the abstinent methamphetamine abusers. A simple analysis showed a significant between-group difference in P3 peak value in the parietal area （PZ electrode）. There was a statistically significant between-group difference in P3 peak value in the 1-back load condition （P〈0.05） and no significant between-group difference in the 2-back load condition in the abstinent methamphetamine abusers before and after acupuncture treatment. The N450 component of the difference waves was distributed mainly in the parietal area and extensively in the brain regions. Conclusion The PZ electrode is an electrode with which a more obvious P3 component is recorded in drug-related N-back tasks. Through acting on this site of memory, acupuncture may produce a therapeutic effect on methamphetamine-induced dysmnesia.
