经超声心动图诊断右心室双出口368例与手术结果对比分析

摘　　要：目的 :探讨超声心动图在右心室双出口(DORV)诊断中的价值。方法 :回顾368例经双心室矫治术证实的DORV,对照手术结果,分析术前超声诊断情况。结果 :超声定性诊断准确率88.9%(327/368),误诊率5.4%(20/368),漏诊率5.7%(21/368);对于室间隔缺损(VSD)与大动脉关系、大动脉位置关系和动脉骑跨、超声分型诊断总符合率分别为:70.2%、76.2%和72.5%。结论 :超声心动图能够准确的定性诊断DORV;对于病变程度复杂、病理类型相对少见的DORV,超声分型诊断水平仍需提高。Objective： To evaluate the diagnostic value of echocardiography in patients with double outlet right ventricle （DORV）. Methods： We retrospectively studied 368 patients with corrective surgery confirmed DORV, and compared the findings between preoperative echocardiography and surgery. Results： Based on the result of operation, the diagnostic accuracy of echocardiography for DORV was 88.9% （327/368）, the misdiagnosis was 5.4% （20/368） and the missed diagnosis was 5.7% （21/368）. The coincidence rate of echocardiography for the relationship between ventricular septal defect and great artery was 70.2%, the coincidence for the position of great artery was 76.2%, and for the overriding of great artery was 72.5% respectively. Conclusion： The Echocardiography could correctly diagnose DORV, while the diagnostic classification should be improved for the patients with complicated lesions and rare pathological changes.
