多功能穿刺针的研制及其临床应用

摘　　要：目的 研制新型多功能穿刺针,提高临床诊疗成功率和安全性。方法 研制的多功能穿刺针(外径1.2mm)由针鞘和针芯二部分构成。针鞘头端是45度斜角并设有多个侧孔,底部可与标准注射器连接。针芯特制有二根,针芯1与针鞘配合主要用于液性目标的穿刺抽吸;针芯2与针鞘配合则用于实性目标的组织活检。结果 完成106例,共128次颈、胸和腹等部位的穿刺诊疗操作,穿刺成功率为100％(106/106)。全组病变物理性质属实性和混合性者计69例,穿刺取材成功率为95.7％(66/69)。其中,所取标本可供病理切片检查明确诊断者占92.4％(61/66)。本组病例未发生气胸、内出血和感染等重要穿刺并发症。结论 多功能穿刺针适用于实性和液性目标的穿刺诊疗术,具有安全可靠,一针多用的优点。Purpose To develop a new multipurpose puncture needle to improve the security and efficiency of clinical practice. Methods The newly developed multipurpose puncture needle with outer diameter of 1.2mm is composed of two parts: tubing and stylet.The tip of the tubing was cut into oblique angle of 45 with several eye holes. The end of the tubing could be connected with the normal injector. There were two special stylets made for the needle; stylet I was used with needle for cytologic aspiration biopsy and stylet II for histologic biopsy. Results A total of 128 biopsies for 106 cases of neck,chest and abdomen had been performed with a 100 percent success rate (106/106). In 69 cases of solid or mixed lesions in physical characteristic classification, the success rate of obtaining sample was 95.7% (66/69). Among them, 92.4% (61/66)were clearly diagnosed with samples used for pathologic examination. No complication such as entorrhagia, pneumothorax and infection had occurred due to puncture biopsy. Conclusion The new puncture needle is suitable for both cytologic aspiration biopsy and histologic biopsy for its safety and multipurpose.
