对声学定量技术测定小儿扩张型心肌病患者左室功能的评价

摘  要:目的 :探讨声学定量 (AQ)技术检测小儿扩张型心肌病患者的左室功能的应用价值并评价患儿的心功能改变。方法 :应用声学定量技术、二维越声心动图 (2 - DE)及多普勒越声心动图 (PDE)检测 16例扩张型心肌病 (DCM)患儿的左室功能 ,并对两种方法进行对比研究 ,探讨二者间的一致性。结果 :本组患儿 93%可进行 AQ技术检查 ;后者可取代 2 - DE测定小儿 DCM的左室面积指标、射血分数 ,但两者测定的左室容积指标一致性尚欠佳 ;DCM患儿的左室舒张末期及收缩末期面积增大 ,面积变化分数、射血分数降低 ,显示左室收缩功能受损 ;对扩张型心肌病患儿左室舒张功能检测表明 ,AQ技术敏感性高于 PDE,尤其 PDE二尖瓣血流频普显示假性正常时。AQ技术与 PDE的联合应用 ,更有利于小儿左室舒张功能的检测。结论 :AQ技术可实时、快速、可靠地进行小儿 DCM心功能检测 ,为临床药物的选择。Object:This study was to set up normal values of Acoustic Quantification(AQ) in children who suffered from dilated cardiomyopathy(DCM) and to compare the AQ with two dimensional echocardiography. Method:The left ventricular function of 16 children of DCM was studied by two dimensinal echocardiography (2 DE) and AQ. The measurements of left function were compared between the two methods. Result:93% patients of DCM had a technically adequate conventioinal echocardiograpy. End diastolic area,end systolic area,area change fraction and ejection fraction which were measured by these two methods were consistent,but the measurement of left ventricular diastolic end volume and systolic end volume of AQ were under estimated comparing with that of 2 DE. Criteria for abnormal relaxation were present in 80% by AQ and in 20% by Doppler echocardiography(PDE). Abnormal relaxation was detected by AQ while normal presence in PDE. Conclusion:The AQ provides a new,clinical useful method for real time estimation of the left ventricular function. But it is associated with cavity volume measurement errors that need to be addressed.
