摘  要:目的 :探讨己酮可可碱 (PTX)对创伤性急性肺损伤 (AL I)的防治作用及可能的机制。方法 :采用兔创伤性 AL I模型 ,观察 PTX对动脉血氧分压、血浆和支气管肺泡灌洗液 (BAL F)中肿瘤坏死因子 α(TNFα)含量及 BAL F中白细胞数量、白蛋白含量的影响。进行肺组织的病理学光镜检查 ,用原位杂交方法结合原位定量分析检测肺、肝、心、肾组织中 TNFα的 m RNA表达。结果 :PTX治疗组与模型组相比较 ,血氧分压改善 (P<0 .0 5 ) ,血浆和 BAL F中的 TNFα含量明显降低 (P<0 .0 1或 P<0 .0 5 ) ,肺泡、支气管中的白细胞浸润减少(P<0 .0 5 ) ,白蛋白含量降低 (P<0 .0 5 )。病理观察 :肺组织损伤程度减轻、肺湿重 /干重 (W/ D)值下降。TNFαm RNA在肺等组织的巨噬细胞、中性粒细胞和血管内皮细胞中表达数量不同程度减少 (P均 <0 .0 5 )。结论 :PTX在一定程度上能有效改善创伤性 AL I的发生和发展。Objective:To investigate the preventive and therapeutic effects of pentoxifylline(PTX) on traumatic acute lung injury(ALI) and their potential mechanism.Methods:Rabbit models of ALI were used to observe the effect of PTX on arterial oxygen partial pressure,and changes in tumor necrosis factorα(TNFα) contents both in plasma and bronchoalveolar lavage fluid (BALF),white blood cell counts and the albumin contents in BALF.The histological features of lung were examined by the naked eyes and light microscope.In situ hybridization combined with in situ quantitative analysis was also used to detect the TNFα mRNA expression in lungs,liver,heart as well as kidneys.Results:Compared with ALI group,PTX treatment could significant improve the arterial oxygen partial pressure(P<0 05),decrease the TNFα contents of plasma and BALF( P<0 01 or P<0 05).The white blood cell counts(P<0 05) and albumin contents(P<0 05) in BALF were also decreased in PTXtreated group.Pathological examination revealed a marked improvement in lung injury with decreased wet/dry weight of lungs.The TNFα mRNA expression of macrophage,polymorphonuclear neutrophils and vascular endothelial cell in pulmonary tissue markedly decreased(all P<0 05).Conclusions:The results suggested that PTX can significantly prevent the development of traumatic ALI.
