摘  要:目的评价血浆BNP增高对慢性阻塞性肺病(COPD)急性加重患者预后的影响。方法对2011-2012年我院COPD住院患者资料进行回顾性研究,入选40名连续入院的AECOPD患者及40名稳定期的COPD患者。入选患者均进行了BNP检测。入选患者的临床、影像学检查、血气分析、肝功能测定及心脏彩超检查进行分析。结果急性加重COPD患者血浆BNP水平(950±473)明显高于稳定期患者(263±52)(P<0.05)。BNP水平增高与肺功能及低氧血症无明显关系。血浆BNP与射血分数(P<0.05)及肺动脉收缩压有明显关系(P=0.004),血浆BNP增高与患者死亡率增加有明显联系。结论高血浆BNP水平是COPD患者病情加重的危险因素,并且增加了患者的死亡危险,提示COPD预后不良。[ Objective ] We aimed to evaluate the effect of plasma brain natriuretic peptide (BNP) elevated on patients with COPD acute exacerbation. [ Methods ] Retrospective cohort study with analysis of admissions for acute exacerbation of COPD in 2011-2012. We selected 40 patients with AECOPD and 40 patients with COPD. All patients had measurement of BNP performed in hospital. Clinical and historical data, arterial blood gas analysis, lung function, Echocardiography were also determined. In-hospital mortality were analysised. [ Resluts ] Plasma BNP in AECOPD patients (950 ± 473)(P〈0.05 ) was significantly higher than in stable COPD(263 ± 52).No sigificant correlations were observed between plasma BNP level and pulmonary function or hypoxia. There was a significant correlation between plasma BNP level and % ejection traction(P〈0.05)and pulmonary artery systolic pressure(P=0.001).Plasma BNP elevated in AECOPD. Elevated BNP levels was significantly associated with high mortality. [ Conclusion ] A high plasma BNP level was not only shown to be an independent risk factor for COPD exacerbation, but also increased prognosis in COPD. risk of death.
