摘  要:目的:研究重组肠促胰岛素分泌肽对单用磺脲类(SUs)降糖药控制不佳的2型糖尿病(T2DM)患者的治疗效果及其对B细胞功能和胰岛素敏感性的影响。方法:纳入单用SUs降糖药治疗3个月以上血糖控制不佳的T2DM患者85例,早晚餐前皮下注射重组肠促胰岛素分泌肽治疗16周。采用胰岛B细胞功能指数(HOMA-B)及早期胰岛素分泌指数(ΔI30/ΔG30)评估B细胞功能;正常血糖胰岛素钳夹试验评估胰岛素敏感性。测定治疗前后空腹血糖(FPG)、餐后2 h血糖(2hPG)、糖化血红蛋白(HbA1c)等。结果:与治疗前比较,肠促胰岛素分泌肽治疗16周后,FPG、2hPG及HbA1c均明显下降(P<0.01);ΔI30/ΔG30、HOMA-B以及胰岛素敏感指数(M/I)均显著升高(P<0.01)。结论:重组肠促胰岛素分泌肽能显著改善胰岛B细胞功能,增强胰岛素敏感性,有效治疗单药治疗疗效不佳的T2DM。OBJECTIVE:To investigate the effects of recombinant incretin peptide on poor control of blood glucose in patients with type 2 diabetics mellitus(T2DM) by sulfonylureas(SUs) alone,as well as B-cell function and insulin sensitivity.METHODS:85 T2DM patients with poor control of blood glucose after more than 3 months of SUs treatment were included.They were given hypodermic injection of recombinant incretin peptide before breakfast and supper for 16 weeks.B-cell function was evaluated by HOMA-B and early insulin secretion index(ΔI30/ΔG30);insulin sensitivity was evaluated by euglycemic insulin clamp test.The fasting blood glucose(FPG),2 h postprandial plasma glucose(2hPG)and glycosylated hemoglobin(HbA1 c)were all tested before and after treatment.RESULTS:Compared with baseline levels,FPG,2hPG and HbA1 c were all significantly reduced after treatment of recombinant incretin peptide for 16 weeks(P0.01);ΔI30/ΔG30,HOMA-B and insulin sensitivity index(M/I) were all improved significantly(P0.01).CONCLUSIONS:Recombinant incretin peptide could improve B-cell function,enhance insulin sensitivity and make better glucose control after monotherapy failure.
