生血灵治疗激素抵抗型免疫性血小板减少症53例临床观察

摘　　要：目的:探讨生血灵治疗激素抵抗型免疫性血小板减少症(ITP)患者临床治疗效果。方法:选择激素抵抗型ITP患者53例,辨证分为脾肾阴虚组30例和脾肾阳虚组23例,分别服用健脾滋肾作用的Ⅲ号生血灵冲剂(黄芪、党参、当归、生地、熟地、熟女贞、鳖甲、丹皮、仙鹤草、大青叶、苏梗、甘草等)和健脾温肾作用的Ⅳ号生血灵冲剂(黄芪、党参、当归、生地、熟地、淫羊藿、菟丝子、丹皮、仙鹤草、苏梗、甘草等),治疗3个月,观察临床疗效、出血分级、血小板计数和不良反应等指标。结果:生血灵治疗激素抵抗型ITP患者53例,获显效6例、良效12例、进步31例、无效4例,近期总有效率92.44%,其中,脾肾阳虚型疗效明显优于脾肾阴虚型(P<0.05);生血灵可以改善患者出血和中医证候积分,临床未见明显不良反应。结论:生血灵治疗激素抵抗型ITP疗效良好,临床未见明显毒副反应,其中脾肾阳虚型疗效明显优于脾肾阴虚型,既有体现中医学特色的辨证论治,也有辨病与辨证结合的专方专药思路,具有方法简便、不良反应少等特点,值得推广。Objective：To observe the clinical efficacy of Shengxueling in the treatment of hormone refractory immune thrombocytopenia.Methods：53 Steroid-resistant ITP patients were divided into 30 cases of spleen and kidney yin deficiency and 23 cases of spleen and kidney deficiency,respectively,taking the role of nourishing spleen and kidney No.Ⅲ of Shengxueling Granules（Huangqi,Dangshen,Danggui,Shengdihuang,Shudihuang,Biejia,Dangpi,Xianhecao,Daqingye,Sugeng,Gancao,etc.）and the role of warming spleen and kidney No.Ⅳ Shengxueling Granules（Huangqi,Dangshen,Danggui,Shengdihuang,Shudihuang,Xianlingpi,Tusizi,Dangpi,Xianhecao,Sugeng,Gancao,etc.）treatment with 3 months.Clinical effects,bleeding grade,platelet counts and negative reactions were observed.Results：Shengxueling treatment of 53 cases steroid-resistant ITP patients,6 cases were markedly effective,good effect in 12 cases,progress in 31 cases,4 cases in vain.Recently the total effective rate was 92.44%.Among them,the efficacy of spleen and kidney yang deficiency was significantly better than the group of yin deficiency（P〈0.05）.Shengxueling can improve the symptom score and had no significant clinical adverse reactions.Conclusions：Shengxueling treatment of steroid-resistant ITP has good curative effect and no significant clinical toxicity.Spleen and kidney yang deficiency has significantly better effects than the spleen kidney yin deficiency.Both the syndrome characteristics of TCM,also has the special prescription combination of disease differentiation and syndrome differentiation of the special medicine thought.This technique has the characteristics of simple and few adverse reaction,and is worth promoting.
