芪苈强心胶囊对照西药治疗慢性心力衰竭疗效比较的系统评价

摘　　要：目的评价芪苈强心胶囊与地高辛等西药比较治疗慢性心力衰竭的疗效差异及安全性。方法纳入7项随机对照试验(RCT),包括605例慢性心力衰竭患者。采用RevMan 5.1软件进行Meta分析。结果芪苈强心胶囊组对照西药组提高总有效率:优势比(OR)(fixed)=2.91,95%CI[1.68,5.05](P<0.01);提高左室射血分数(LVEF):加权均改善(WMD)(random)=5.57,95%CI[0.99,10.15](P<0.05);提高左室功能指标E峰WMD(fixed)=10.31,95%CI[7.01,13.61](P<0.01),E/A:WMD(fixed)=0.28,95%CI[0.03,0.53](P<0.05);降低血浆内皮素(ET):WMD=-26.9,95%CI[-36.56,-17.24](P<0.01);提高一氧化氮(NO):WMD=43.90,95%CI[28.20,59.60](P<0.01);提高降钙素基因相关肽(CGRP):WMD=19.40,95%CI[15.37,23.43](P<0.01);提高反应性充血状态下肱动脉内径变化率(1):WMD=5.40,95%CI[4.34,6.46](P<0.01)。3篇论文报道未发现芪苈强心胶囊明显毒副作用,不良反应少。结论现有证据提示芪苈强心胶囊治疗慢性心力衰竭优于一线西药,安全有效。Objective To evaluate the differences in curative effect on chronic heart failure between Qili Qiangxin capsule and western medicine like digoxin, and their safety. Methods A meta - analysis of seven random- ized controlled trials（RCTs） encompassing 605 chronic heart failure patients were conducted. Results （~） Test of the combined curative effect of 6 studies showed an OR affiliate of 2.91,95%, and the confidence interval of OR was [1.68,5.05]（P〈 0.01）. （~）Raise left ventricular ejection fraction（WMD = 5.57[0.99,10.15] （P〈 0.05）. （~） Improve left ventricular function parameters E peak （ WMD = 10.31, [ 7.01,13.61 ] （ P 〈 0.05 ） and E/A （ WMD = 0.28, [0.03,0.53 ] （P 〈 0.05 ）. （~）The levels of ET was decreased and the level of NO and CGRP were raised. Change rate of brachial artery diameter under reactive hyperemia was raised WMD = 5.40 [ 4. 34,6.46 ] （ P 〈 0.01 ）. （~）Three papers reported that Qili Qiangxin capsules found no obvious toxic and side effects. The Qili Qiangxin capsules number of adverse drug reactions was less than that of Western medicine. Conclusion Existing evidence suggests that the effect of Qili Qiangxin capsule was superior to that of first - line medicine on the treatment of chronic heart failure and safe better than western medicine.
