手部皮肤撕脱伤治疗方法选择

摘　　要：目的探讨手部皮肤撕脱伤治疗方法选择。方法对51例手部皮肤撕脱伤患者急诊行手部撕脱皮肤回植、吻合血管,对损伤的骨折及肌腱同期修复,部分皮肤条件差者待Ⅱ期行皮瓣修复或游离植皮覆盖创面。结果 51例手部皮肤撕脱伤患者有32例皮肤Ⅰ期全部成活,皮肤坏死5例,部分坏死14例,Ⅰ期扩创不能闭合或伴有重要组织外露者经换药、植皮、皮瓣修复,均取得满意疗效。结论对于手部撕部伤,应根据撕脱皮肤本身条件以及撕脱皮肤下软组织条件、是否合并骨骼、肌腱外露,采取原位缝合、吻合血管、游离植皮或皮瓣转移的方法来修复创面,能取得良好效果。Objective To explore the choice of diagnosis and treatment of avulsion injury of hand skin. Methods 51 patients with degloving injury of hand skin line emergency hand avulsed skin implant, vascular anastomosis ,the damage of fracture and tendon repair. Some memory, skin conditions for phase ii line flap or free skin graft to cover the wound. Results 32 patients with degloving injury of hand skin issue all survived, skin necrosis was 5 cases, partial flap necrosis was 14 cases. The issue of debridement could not be closed or accompanied by important organization exposed by the treatment, skin graft, skin flap repair, had achieved satisfactory curative effect. Conclusion For the hand to tear injury,we should according to the avulsed skin itself when processing conditions and under the avulsed skin soft tissue, bone and tendon exposed whether merger, suture in situ, vascular anastomosis, free skin graft or skin flap transfer method to repair wounds, and achieves good results.
