微创技术治疗胫骨干骺端骨折

摘　　要：目的探讨微创经皮钢板内固定(MIPPO)技术在胫骨骨折治疗中的临床疗效。方法选取胫骨骨折56例,包括胫骨近、远端骨折,均行经皮微创内固定。结果手术时间30～130 min,平均70 min;失血量150～400 mL,平均250 mL。所有患者无骨折延迟愈合及畸形愈合,术后患者软组织肿胀轻微,伤口均Ⅰ期愈合。骨折临床愈合时间平均为11.6周,X线片显示胫骨平台、踝关节均未发生延迟复位丢失,无下肢轴线不良。膝踝关节功能评分,优44例,良4例,中3例,差1例。结论微创技术在治疗胫骨近、远端骨折上具有创伤小、手术时间短、并发症少、费用低等优点,可在熟练掌握骨折内固定技术及适应证的情况下广泛开展。Objective To evaluate the efficacy of minimally invasive percutaneous plate osteosynthesis （MIPPO） for the treatment of tibial fracture. Methods 56 patients with tibial fracture were treated with MIPPO in our hospital, the facture was located at the proximal tibia in 24, the distal tibia in 29, and the shaft in 3cases. Results The operation time lasted 30 - 130 minutes （average time, 70 minutes） ;the intra operative blood loss ranged from 150 to 400 ml （average blood loss,250ml） ;all the incisions healed primarily. The fractures healed within 11. 6 weeks after the operation. No complications, including delayed incision healing,incision infection, pa- renchymal necrosis, delayed union and non - union, occurred in the series. The knee and ankle functions were excellent in 44 patients, good in 4 patients, fair in 3 patients, and poor in one patient. Conclusion The MIPPO provides biological fixation and results in minimal parenchymal injury in patients with tibial fracture. By using the technique, the patients recover quickly with few complications and mild reaction to the trauma.
