摘  要:目的獉獉:探讨氯胺酮所致膀胱挛缩的临床特点及治疗方法。方法獉獉:回顾性分析2005年1月-2013年1月武汉大学人民医院泌尿外科收治的21例长期吸食氯胺酮致膀胱挛缩患者的诊疗资料。根据常规检查及泌尿系相关检查结果,给予患者不同的探讨性治疗。结果獉獉:所有患者停用氯胺酮,选择性给予系统药物治疗(抗生素、肾上腺素能受体阻滞剂、胆碱能受体阻滞剂,戊聚硫钠和透明质酸钠膀胱灌注)和/或膀胱水扩张术。半月到2月后症状缓解及膀胱容量较前扩大18例,3例患者经上述治疗无好转,行乙状结肠膀胱扩大术后症状缓解。治疗后,尿流动力学显示:最大尿流率(16.5±s 2.8)ml.s-1,膀胱容量(178±s 28)ml,残余尿量(38±s 9)ml;PUF评分平均16.3(15-24)分。结论獉獉:对氯胺酮所致膀胱挛缩,药物治疗可取得较好效果,对药物治疗无效者,可尝试行乙状结肠膀胱扩大术。Objective: To investigate the clinical characteristics and treatments of contracted bladder induced by ketamine. Methods: Clinical data of 21 cases suffering from contracted bladder induced by ketamine from 2005 January to 2013 January were collected and analyzed retrospectively. According to the routine examination and urinary examination, different therapies were attempted. Results:All patients stopped ketamine abuse and selectively received systemic medication (antibiotics, adrenergic receptor blockers, cholinergic receptor blockers, pentosan polysulfate sodium and sodium hyaluronate intravesical therapy ) and/or bladder pressure expansion. Eighteen cases relieved the symptoms and the bladder capacities were expanded. Three patients without improvement after the above treatment, patients accepteing sigmoid cystoplasty whose symptoms were relieved. After treatment, Qmax increased to ( 16.5 ± s 2.8 ) ml·s-1, bladder capacity volume reduced to (38 + s 9)ml, the average PUF score therapy is an effective way in the treatment of katamine invalid, sigmoid cystoplasty could be attempted. increased to (178 ± s 28 )ml, residual urine was 16.3 ( 15 - 24).
