摘 要:目的:对广东汉族妊娠糖尿病(GDM)患者进行钙蛋白酶10(Calpain10)基因单核苷酸多态性(SNP-43、-19、-63),及其胎盘组织Calpain10基因产物的检测,探讨Calpain10基因易感单倍型组合GDM罹患率增高的可能相关机制。方法:选择60例正常孕妇(NGT)和60例GDM患者,PCR-RFLP法检测Calpain10基因SNP-43、-19、-63位点,统计并分析其易感单倍型组合。RT-PCR法检测研究对象剖宫产后胎盘组织中Calpain10mRNA的表达,分析其与Calpain10基因易感单倍型组合的相关性。结果:①Calpain10基因单倍型组合112/112(SNP-43、-19、-63)的广东汉族孕妇有较高的GDM罹患率(χ2=4.324,P=0.037)。②与其他单倍型组合个体相比较,112/112个体胎盘组织(t=2.135,P=0.035)中Calpain10 mRNA表达明显下降。③112/112个体孕期体重指数变化(t=2.153,P=0.033)及胰岛素抵抗水平(t=2.189,P=0.031)明显增加。结论:Calpain10基因单倍型组合为112/112的妇女有较高的GDM发患率,这可能与通过增加孕期体重以及胎盘组织Calpain10表达减少,进而增加胰岛素抵抗水平相关。Objective: To detect single nucleotide polymorphism ( SNP - 43, - 19, - 63) of calpain 10 gene and products of cal- pain 10 gene in placental tissue of patients of Han nationality with gestational diabetes mellitus (GDM) in Guangdong province, explore the probable mechanism of increase of prevalence rate in patients with susceptible haplotype combination of ealpain 10 gene. Methods: Sixty normal pregnant women and sixty GDM patients were selected, PCR - RFLP was used to detect SNP -43, - 19, -63 loci of ealpain 10 gene, the susceptible haplotype combination was analyzed statistically. RT - PCR was used to detect the expression of ealpain 10 mRNA in placental tissue of study objects, and its correlation with susceptible haplotype combination of ealpain 10 gene was analyzed. Results: The prevalence rate of GDM in pregnant women of Han nationality in Guangdong province with haplotype combination 112/112 (SNP -43, - 19, - 63 ) of calpain 10 gene was relatively high (X^2 = 4. 324, P = 0. 037 ) . Compared with cases with other haplotype combination, the expression level of ealpain 10 mRNA in placental tissue of women with haplotype combination 112/112 of ealpain 10 gene decreased signifi- candy (t =2. 135, P =0. 035), the body mass index (BMI) during gestational period and insulin resistance level in creased significantly (t =2. 153, P =0. 033) (t = 2. 189, P = 0. 031 ) . Conclusion: The prevalence rate of GDM in pregnant women of Han nationality in Guangdong province with haplotype combination 112/112 of ealpain 10 gene was relatively high, which may be correlated with enhancing in- sulin resistance by increasing gestational weight and reducing ealpain 10 expression in placental tissue.
