左炔诺孕酮宫内缓释系统预防子宫内膜息肉复发的疗效观察

摘　　要：目的:观察宫腔镜下息肉摘除术联合左炔诺孕酮宫内缓释系统治疗子宫内膜息肉的疗效,探讨预防子宫内膜息肉复发的治疗方案。方法:选取经宫腔镜检查并经术后病理证实为子宫内膜息肉的患者102例,随机分为3组,研究组①32例,宫腔镜下内膜息肉摘除术后放置左炔诺孕酮宫内缓释系统;研究组②41例,宫腔镜下内膜息肉摘除术后于月经后半周期口服黄体酮10天;对照组29例,宫腔镜下内膜息肉摘除术后无任何处置。比较3组患者6个月后子宫内膜厚度及子宫内膜息肉复发率。结果:与对照组比较,研究组①、研究组②子宫内膜息肉复发率较低(P<0.05),内膜变薄(P<0.05);研究组①与研究组②子宫内膜息肉复发率比较差异无统计学意义(P>0.05),但治疗后内膜厚度比较差异有统计学意义(P<0.05)。结论:宫腔镜下息肉摘除术联合左炔诺孕酮宫内缓释系统能有效预防子宫内膜息肉复发。Objective： To observe the curative effect of hysteroscopic polypectomy combined with levonorgesterel - releasing intrau- terine system （LNG- IUS） in treatment of endometrial polyps, explore the treatment programs to prevent recurrence of endometrial polyps. Methods： A total of 102 patients diagnosed as endometrial polyps by hysteroscopy and postoperative pathological examination were selected and randomly divided into three groups ; 32 patients in study one group were treated with LNG - IUS after hysteroscopic polypectomy, 41 pa- tients in study two group were treated with hysteroscopic polypectomy and oral administration of progesterone during the latter half menstrual cycle for ten days ; 29 patients in control group were treated with hysteroscopic polypeetomy. The thicknesses of endometrium and recurrence rates of endometrial polyps at six months after treatment in the three groups were compared. Results： Compared with control group, the re- currence rates of endometrial polyps in study one group and study two group were lower （ P 〈 0. 05 ） , and the thickness of endometrium was thinner （ P 〈 0. 05 ） ; there was no statistically significant difference in the recurrence rate of endometrial polyps between study one group and study two group （ P 〉 0. 05 ）, but there was statistically significant difference in thickness of endometrium after treatment between study one group and study two group （ P 〈 0. 05 ） . Conclusion ： Hysteroseopic polypectomy combined with LNG - IUS can effectively prevent recur- rence of endometrial polyps.
