摘 要:目的探讨冬季出生婴儿维生素D补充剂量对血清25羟维生素D[25(OH)D]的影响。方法2009年12月至2010年3月首都儿科研究所附属儿童医院采集69名北方冬季出生的新生儿脐带血及补充维生素D4个月时的静脉血,采用酶联免疫吸附法检测血清25(OH)D,全自动生化分析仪测定静脉血钙、磷、碱性磷酸酶并分析。结果血清25(OH)D随维生素D摄入量增加而增加。维生素D每日摄入量中位数为39.5、190.5、218.0、290.5、421.0、575.0IU时,婴儿血清25(OH)D〈27.5、27.5~〈50.0、50.0-〈75.0、75.0~〈125.0、125.0—170.0、≥170.0nmol/L。每日维生素D摄入中位数为387.0IU时,婴儿血清25(OH)D≥75.0nmol/L。血清钙、磷、碱性磷酸酶差异无统计学意义(P〉0.05)。血清25(OH)D与喂养方式有关,母乳喂养婴儿低于非母乳喂养婴儿(×。=8.911,P〈0.05)。结论为使婴儿血清25(OH)D达充足水平,新生儿应在出生后15d内开始补充维生素D。北方冬季日照不足,维生素D每日摄入量应增加。Objective To evaluate the effects of vitamin D supplementation on serum 25 (OH)D level in newborn infants in the northern part of China during winter. Methods Totally 69 healthy neonates were recruited for the study. Blood samples were obtained from neonates (cord blood) and infants (vein blood) ;the neonates received vitamin D supplement from 15 days to 4 months after birth. Serum 25 (OH)D concentrations were determined by ELISA.Serum calci- um, phosphate and ALP levels were analyzed. Results There were clear dose-related increments in serum 25 (OH)D with increasing supplemental vitamin D.When the daily vitamin D intake was 39.5,190.5,218.0,290.5,421.0 and 575.0 IU, the serum 25 (OH) D level was 〈 27.5,27.5 - 〈 50,50 〈 75,75 -〈 125,125 - 170 and 1〉 170nmol/L, respectively. Vitamin D intake should at least achieve 387 IU/d if the serum 25 (OH)D concentration was I〉 75 nmol/L. No significant changes were found in serum calcium, phosphate and ALP levels in infants. Rank sum test showed that there was a rela- tionship between 25 (OH)D concentration and feeding pattern, serum 25 (OH)D concentration was lower in breast-fed infants than non-breast-fed infants (P 〈 0.05 ). Conclusion It is recommended that all infants should have a daily in- take of vitamin D to maintain the optimal level of 25 (OH)D beginning after birth within 15 days. In northern China, sun exposure is limited during the winter months, supplementation of vitamin D should be appropriately increased.
