不同病程咳嗽患者证素特征研究

摘　　要：目的:采用聚类分析探讨咳嗽患者不同病程的证素特征。方法:通过大样本临床调查研究,收集1 095例咳嗽患者的四诊信息,运用频数及聚类分析方法研究不同病程咳嗽患者的证素特征。结果:咳嗽常见病位证素为肺、表、脾、肾、心、肝。痰是各病程组咳嗽的主要病性。肺贯穿着咳嗽的始终,咳嗽不离于肺,病位表可出现各个病程阶段;痰是咳嗽主要的病理基础,六淫以风、寒、热最多,热占主导地位。结论:不同病程,虽然出现的病位及病性证素相同,但组合却不尽一致,而且病程越长,其所涉及到的病位证素与病性证素越多,证素组合越复杂。提示采用固定的证型研究病具有很大的局限性。Objective： The character of syndrome elements of patients with different duration of cough were analyzed by using cluster analysis. Methods： Through large sample of clinical research study, four diagnostic information of 1 095 cough cases were collected, the character of syndrome elements of different duration were analyzed by using frequency and cluster analysis.Results： The common disease location syndrome elments of cough were lung, superficies, spleen, kidney, heart and liver. Phlegm was the main disease nature syndrome elment of cough across different duration groups. According to disease location table, lung is the main disease location syndrome element of cough, which appeared in various stages of disease duration; phlegm was the pathological basis of cough through the course of disease, the main etiological factors of the six climatic exopathogens were wind, cold and heat, and the last is the dominant. Conclusion： Although the syndrome elements of disease location and nature are the same in different duration, the combination is not consistent, and the longer the duration is, the more location and nature elements involved in and the combination of them are more complex. This suggests that using the method of fixed syndrome types to study disease has serious limitations.
