摘  要:目的探讨不同血糖水平对2型糖尿病(Type 2 Diabetes Mellitus,T2DM)整体认知功能及认知功能域的影响。方法对130例血糖控制不良2型糖尿病进行为期12个月的严格降糖治疗及随访,比较治疗前后认知功能差异。结果①2型糖尿病与正常对照组的认知功能差异有统计学意义(P<0.001),其视空间、语言、注意力、延迟回忆等方面显著下降(P<0.001);②严格降糖治疗后6个月随访,2型糖尿病总体认知功能改善(P<0.05),随访12个月时视空间及注意力有显著改善(P<0.05),但语言及延迟记忆功能无改善。结论 2型糖尿病具有独立于脑血管病的认知功能损害,严格降糖治疗可对总体认知功能机部分认知功能域具有改善作用。Objective To explore the influence of the cognitive function and cognitive domainfunction with different levels of blood glucose in type 2 diabetes mellitus( T2DM) . Methods 130 cases were given glucose-lowering therapy with strict control,and all the cases were followed up 12 months,comparing the differences in cognitive function before and after the treatment. Results ① The visual-spatial,language,attention and delayed recall were significantly declinedin T2DM than the control( P 0. 001) . ②The attention and visual-spatial has improved significantly while language and delayed memory function has not when 12 months follow-up ( P 0. 05) . Conclusion Cognitive dysfunction wasexisted in T2DM before cerebrovascular damages,especially in patients whose blood sugarvalueswere not under control. And to regulate the level of blood glucose effectively should improve parts of cognitive domain function.
