^(99)Tc^m-MIBI显像联合血清甲状旁腺素测定对原发性甲状旁腺机能亢进症的诊断价值

摘　　要：目的:对临床怀疑原发性甲状旁腺功能亢进症(PHPT)患者行99 Tcm-MIBI显像及血清甲状旁腺素(PTH)测定,阐明99 Tcm-MIBI显像联合PTH测定对PHPT的诊断价值。方法:回顾性分析43例怀疑PHPT患者99 Tcm-MIBI显像及血清PTH测定结果,以术后病理为金标准,对99 Tcm-MIBI显像、血清PTH测定及99 Tcm-MIBI显像联合血清PTH测定3种诊断方法的灵敏度、特异度、阳性预测值、阴性预测值、阳性似然比及阴性似然比、准确度进行比较和评价。结果:以术后病理证实,99 Tcm-MIBI显像、血清PTH测定及99 Tcm-MIBI显像联合血清PTH测定3种诊断方法比较,灵敏度分别为93.75%、84.38%及100.00%,特异度分别为45.50%、54.55%及36.36%,阳性预测值分别为83.33%、84.38%及88.89%,阴性预测值分别为71.43%、54.55%、100.00%,阳性似然比分别为1.72、1.85及1.57,阴性似然比分别为0.14、0.29及0,准确度分别为81.40%、76.74%及83.72%。结论:99 Tcm-MIBI显像联合血清PTH测定可以提高PHPT的诊断准确性,降低漏诊率。Objective To analyze the results of 99 Tcm-MIBI imaging and parathyroid hormone（PTH）measurement of the patients with suspected primary hyperparathyroidism（PHPT）and to evaluate the diagnostic value of 99 TcmMIBI imaging combined with PTH measurement in PHPT.Methods The results of 99 Tcm-MIBI imaging and serum PTH measurement in 43patients with suspected PHPT were retrospectively analyzed.The postoperative pathology was used as the golden standard for PHPT.The sensitivities,specificities,positive predictive values,negative predictive values,accuracies,negative likelihood and positive likelihood of 99 Tcm-MIBI imaging,PTH measurement,99 Tcm-MIBI imaging combined with serum PTH measurement were compared and evaluated.Results The sensitivities,specificities,positive predictive values,negative predictive values,negative likelihood,positive likelihood and accuracies of 99 Tcm-MIBI imaging,PTH measurement and 99 Tcm-MIBI imaging combined with serum PTH measurement were 93.75%,84.38%,100%;45.50%,54.55%,36.36%;83.33%,84.38%,88.89%;71.4%,54.55%,100%;1.72,1.85,1.57;0.14,0.29,0;81.40%,76.74%,83.72%.Conclusion 99 Tcm-MIBI imaging combined with serum PTH measurement can increase the diagnostic accuracy and decrease the rate of missed diagnosis.
