急性下肢深静脉血栓形成经皮下腔静脉滤器置入并溶栓导管留置的疗效分析

摘　　要：目的探讨急性下肢深静脉血栓形成（DVT）患者经数字减影血管造影（DSA）行经皮下腔静脉滤器（VCF）置入并溶栓导管留置治疗的效果。方法2009年1月至2012年9月DVT患者40例，所有患者均经双髂及双下肢静脉彩超确诊后，经股静脉或颈静脉放置VCF，并留置溶栓导管溶栓。结果40例患者VCF均放置成功。本组患者治愈25例，显效13例，有效2例，显效率95％，总有效率100％；平均住院时间（9．11±4．33）d。所有患者溶栓过程中未出现出血及其他并发症。结论DVT患者经DSA行经皮VCF置人并溶栓导管留置是治疗DVT的有效方法之一，并且有利于下肢静脉瓣功能的恢复，有效预防了致命性肺栓塞的发生。Objective To investigate the acute lower extremity deep venous thrombosis （DVT） patients after digital subtraction angiography （DSA） in percutaneous vena cava filter （VCF） placement and treatment effect of thrombolytic catheter. Methods From January 2009 to September 2010, 40 first aid patients who were diagnosed of VCF by color Doppler of bi-ilia and lower extremities underwent percutaneous transluminal VCF and indwelling thrombolytic catheter. Results VCF were successfully grafted to all the 40 patients. Among the patients, 25 were cured, 13 cass had marked effect, and 2 cases improved. The significant efficiency and total effectiveness was 95% and 100% respectively. The average hospital stay was （9. 11 ± 4. 33） d. No bleeding or other complications were observed during thrombolytic therapy. Conclusion DSA first aid DVT percutaneous translumind VCF indwelling thrombolytic catheter is one of the effective methods to treat DVT. Furthermore, it is beneficial to the deep venous valve functional rehabilitation, and can also prevent pulmonary embolism.
