危重症患儿继发胰腺受损的临床研究

摘　　要：目的研究分析危重症患儿继发性胰腺损害的临床特点。方法对2009年6月至2012年6月收治我院儿科PICU的病人,就其年龄、性别、血常规、血钙、血糖、LDH、肾功能、CRP、动态血淀粉酶及胰腺影像学等资料进行研究,根据结果分为胰腺受累组和非受累组,比较两组资料的差异。结果符合研究条件的病人共83例,受累组21例(25.30%),非受累组62例(74.70%)。两组年龄、性别及血WBC、PLT、Ca2+、血糖、BUN、Cr比较,差异均无统计学意义(P>0.05);受累组LDH、CRP高于非受累组(P<0.05);两组血淀粉酶在入院后0h、24h差异无统计学意义(P>0.05),而在48h、72h受累组高于非受累组(P<0.05)。结论危重病人继发性胰腺损伤常见,早期血淀粉酶可能正常,动态监测对诊断继发胰腺受损有重要意义。Objective To analyze the characteristics of the cases with pancreatic damage in critically ill children. Methods We reviewed the clinical data of patients treated in PICU of our hospital from June 2009 to June 2012, including age, gender, blood routine, calciumion, blood glucose, LDH, renal function, CRP, dynamic serum amylase and pancreatic imaging, and di- vided the patients into the involvement group and non-involvement group whether pancreatic damage or not. Results There were 83 patients in the study, the involvement group had 21 patients （25.30%）, the non-involvement group had 62 patients （74.70%）. There was no difference in age, gender, WBC, PLT, calciumion, blood glucose, BUN and Cr between the two groups （P〉0.05）; the levels of LDH, CRP in involvement group were higher than those in non-involvement group （P〈 0.05）. The levels of serum amylase at 0 h and 24 h after hospitalization did not differ significantly between involvement group and non-involvement group （P〈0.05）, and the levels at 48 h and 72 h after hospitalization in involvement group were higher than those in non-involvement group （P〈0.05）. Conclusion Critically ill patients with secondary pancreatic injury is common~ Serum amylase may be normal in the early and it is important to detect serum amylase serially.
