慢性HBV携带者117例肝组织病理与临床特征关系分析

摘　　要：目的通过分析慢性HBV携带者肝组织炎症、纤维化分级与临床、血清学指标及B超脾长径间的关系,寻找影响肝炎和肝纤维化的相关临床特征。方法对117例患者进行肝穿刺并予肝组织病理炎症分期和纤维化分期,同时测定各临床、血清学指标及B超脾长径。根据肝组织炎症分级,以G2为界,分为G2～4和G0～1两组,对两组患者的年龄、性别、HBeAg、HBV DNA定量、脾长径等指标进行比较。结果 117例慢性HBV携带者中有98例存在不同程度的肝组织损伤,炎症分级G2～435例,共占35.7%;其中1例(1.0%)已进展为肝硬化;纤维化分期S2～486例,共占87.8%。年龄≥40岁者所占比例在肝组织炎症分级G2～4组高于分级G0～1组,差异有统计学意义(P<0.05)。脾长径在G0～1组平均值小于G2～4组,差异有统计学意义(P<0.05)。结论慢性HBV携带者大多存在不同程度的肝组织损伤,损伤以轻度为主,中重度损伤也占一定比例,对40岁及以上慢性HBV携带者,应高度重视,以防病变的隐匿进展。Objective By analyzing of chronic HBV carriers of liver tissue inflammation, fibrosis grading and clinical, se- rologic and ultrasound of the relationship between spleen index length-diameter, to explore the relevant clinical feature of hepati- tis and fibrosis. Methods The liver biopsy was conducted in 117 patients in order to confirm liver tissue inflammation pathologi- cal staging （G0-4） and fibrosis stage （S0-4）, and simultaneous determination of clinical, serologic and ultrasound spleen length to diameter. In liver tissue inflammation grading G4-4 and G0-1 was divided into two groups, the two groups of patients＇ age, gender, HBeAg status, HBV DNA quantity and spleen indices such as length to diameter were used in the comparison. Results In 117 cases of chronic HBV carriers, there were 98 cases with varying degrees of liver tissue injury and 35 cases with inflammation grading G2-4 , accounted for 35.7% ; Among them one case （1.0%） had progress to cirrhosis of the liver; There were 86 cases with fibrosis staging acuity S2-4, accounted for 87.8%. The age of 40 or higher proportion in liver tissue inflammation grading G2-4 group was higher than grade G0-1. Splenic length to diameter in the G0-1 averaged less than G2-4 group, with statistical signifi- cance. Conclusion Chronic HBV carriers have varying degrees of liver tissue injury, and light mainly. Chronic HBV carriers of 40 years old and above should pay great attention in order to prevent the progress of hidden lesions.
