摘  要:目的:探讨门控心肌显像相位分析技术在评价病毒性心肌炎(virus myocarditis,VMC)患者左室非同步性及心功能中的应用价值。方法:对21例临床确诊VMC患者和21例正常对照者(healthy control,HC),进行^(99m)Tc-甲氧基异丁基异腈(MIBI)静息门控心肌灌注显像,图像经常规处理重建后应用专用软件(cedars quantitative gated SPECT,QGS)分析,获得相位标准差(phase standard deviation,PSD)、相位直方图带宽(phase histogram bandwidth,PHB)、舒张末期容积(end diastolic volume,EDV)、收缩末期容积(end systolic volume,ESV)及左室射血分数(left ventricular ejection fraction,LVEF)。结果:VMC组与HC组相比,PSD、PHB、EDV和ESV显著升高(P<0.05);LVEF显著降低(P<0.05);PSD或PHB异常的心肌炎患者左室功能明显低于PSD或PHB正常的患者(P<0.05)。结论:门控心肌显像相位分析技术能有效地评价VMC患者左室非同步性,并为可能发生心功能受损的VMC患者提供筛选依据。Aim:To assess the value of phase analysis of gated myocardial perfusion imaging in the evaluation of LV dyssynchrony and function in patients with virus myocarditis (VMC). Methods: Rest 99mTc-MIBI gated myocardial perfusion imaging were performed in 21 patients with VMC and 21 cases of healthy individualy as control. The reconstructed data were submitted to cedars quantitative gated SPECT (QGS) software to measure phase standard deviation (PSD) ,phase histogram bandwidth (PHB) ,end diastolic volume (EDV), end systolic volume (ESV) and LV ejection fraction (LVEF). Results: Compared with VMC patients and healthy group, PSD, PHB, EDV and ESV showed significantly higher and LVEF showed significantly lower; In the VMC group, LV function showed statistically significant difference between patients with LV dyssnchrony and others. Conclusion:Phase analysis of gated myocardial perfusion imaging can effectively evaluate the LV dyssynchrony of VMC patients, moreover, it can provide evidence to select VMV patients whose LV function may deteriorate.
