Impact of Aerosolized Penehyclidine Hydrochloride Inhalation on Intrapulmonary Shunt and Oxygenation During One-lung Ventilation

摘  要:【目的】观察单肺通气(on.1ungventilation,OLV)期间雾化吸入长托宁对肺内分流率(Qs/Qt)、氧合指数(oxygenationindex,0I)和血乳酸(bloodlactieacid,BLA)水平的影响。【方法】选择28例0LV行开胸手术的患者,随机分长托宁组(P组,于0LV期间雾化吸入长托宁)和对照组(C组)。分别于OLV前15min(T0)、OLV15rain(T1)、OLV45min(T2)抽动脉血与混合静脉血行血气分析及BLA测定,计算Qs/Qt与0I。【结果】0LV后两组0I均下降,但P组较c组下降缓慢,且P组Tz较T。有所回升;OLV后两组Qs/Qt和BLA均增加,但P组比C组增加较慢,且P组T2较T1有所下降。【结论】0LV期间雾化吸入长托宁能改善氧舍,降低肺内分流。[Objective]To observe the impact of aerosolized penehyclidine hydrochloride inhalation on in- trapulmonary shunt fraction(Qs/Qt), oxygenation index(OI) and blood lactic acid(BLA) during one-lung ven- tilation(OLV). [Methods]Twenty eight patients with OLV undergoing open chest operation were chosen and randomly divided into penehyclidine hydrochloride group(group P, aerosolized penehyclidine hydrochloride in- halation during OLV) and control group(group C). Arterial and mixed venous blood were drawn for blood gas analysis and BLA detection 15min before OLV(T0), 15min(T~ ) and 45min after OLV(T2 ). Qs/ Qt and OI were calculated. [Results]After OLV, OI in two groups decreased, but that in group P decreased more slowly than that in group C. Compared with T1 , OI in group P at Tz rose again. After OLV, Qs/Qt and BLA in two groups increased, but that in group P increased more slowly than that in group C. Compared with T1 , Qs/Qt and OI in group P at Tz declined again. [Conclusion]Aerosolized penehyclidine hydrochloride inhalation during OLV can improve oxygenation and decrease intrapulmonary shunt.
