关节镜下有限清理配合玻璃酸钠治疗骨性关节炎分析

摘　　要：目的:探讨关节镜下有限清理配合玻璃酸钠对骨性关节炎进行治疗的临床效果。方法:选取骨性关节炎患者132例,随机分为两组,对照组62例,观察组70例,有可比性。对照组患者给予单纯关节镜下关节清理治疗;观察组患者在对照组基础上给予玻璃酸钠腔内注射。使用Lequence评分对两组患者治疗前后关节功能进行评价;并对两组患者治疗效果进行比较。结果:治疗前两组患者Lequence评分差别不大,经过5周治疗,两组患者在治疗1周及5周时Lequence评分与治疗前相比均有下降,且差异明显。在治疗后6个月、12个月、24个月时对照组患者Lequence评分与治疗5周时差别不大(P>0.05),而观察组患者Lequence评分继续降低,明显低于同组治疗5周时Lequence评分,以及同时期对照组Lequence评分(P<0.05)。观察组患者治疗优良率明显优于对照组,两组比较差异明显(P<0.05);两组患者治疗有效率差别不大,无统计学意义(P>0.05)。结论:关节镜下有限清理可以在一定程度上治疗骨性关节炎,但是如能配合玻璃酸钠关节腔内注射,可以取得更好的治疗效果。Objective： To observe the clinical effect of arthroscopic limited cleaning combined with sodium hyaluronate in treating osteoarthritis.Methods： 132 patients with osteoarthritis were selected and divided into two groups randomly,the control group with 62 cases and the observation group with 70 cases.The control group were treated with single arthroscopic joint cleaning,and the observation group were treated with sodium hyaluronate on the basis of the control group.Lequence score was used to evaluate the joint function of two groups before and after treatment,and the treatment effect of two groups were compared.Results： The difference of two groups in Lequence score before treatment was not evident.And 5 weeks after treatment,Lequence score of two groups 1 week and 5 weeks after treatment reduced compared with that before treatment,and the difference was evident（P0.05）.The difference between two groups was not evident（P0.05）.There was no evident difference between Lequence score 6 months,12 months and 24 months after treatment and Lequence score 5 weeks after treatment（P0.05）,and the Lequence score of the observation group continued to reduce,which was lower evidently than Lequence score 5 weeks after treatment of the same group and the control group（P0.05）.The good rate of treatment for the observation group was better than that of the control group,and the difference between two groups was evident（P0.05）.There was no great difference between two groups for the treatment,There＇s no statistical significance（P0.05）.Conclusion： Arthroscopic limited cleaning can treat osteoarthritis effectively,but it can achieve better treatment effect combined with sodium hyaluronate.
