氟比洛芬酯配合舒芬太尼镇痛泵对老年髋关节置换病人术后运动性疼痛的影响

摘　　要：目的探究氟比洛芬酯配合舒芬太尼镇痛泵对老年髋关节置换病人术后运动性疼痛的影响。方法髋关节置换术后老年患者60例,分为Ⅰ组和Ⅱ组,每组30例,两组病人均使用舒芬太尼静脉镇痛泵,术后静息无痛,入ICU2小时后,开始运动试验。实验前30min,Ⅰ组静脉缓慢滴注氟比洛芬酯50mg,Ⅱ组静注生理盐水10ml,观察并记录患者运动后的疼痛视觉模拟评分(VAS)、Ramsay镇静评分及监测开始输注氟比洛芬酯(T1)、输注后10min(T2)、20min(T3)、运动前(T4)、后(T5)5个时点的SP、DP、HR、SpO2。结果病人运动后Ⅰ组和Ⅱ组VAS评分差异明显(P<0.05);Ramsay镇静评分无差异(P〉0.05);与Ⅰ组比较,Ⅱ组HR在T3、T4、T5时点明显升高(P<0.05);SP、DP在T5时点明显升高(P<0.05),两组患者的SpO2无明显变化。结论氟比洛芬酯配合舒芬太尼镇痛泵能够良好的控制老年髋关节置换病人术后运动性疼痛。Objective to investigate the impact of flurbiprofen combined with sufentanil in PCIA on postoperative motion pain of aged total hip replacement patients.Methods 60aged patients undergoing total hip replacement,were randomly devided into groupⅠand groupⅡ,30cases in each.all the patients were given PCIA of sufentanil.there were no rest pain on every patients after operation,the motion study was carried on after admission to ICU 2hours.30minutes before the study,groupⅠwere given flubiprofen 50mg intravenous drip slowly and groupⅡwere given 10ml sodium chloride.the VAS（visual analogue scale）scores,the Ramsay scores were observed and recorded.systolic pressure,diastolic pressure,heart rate and SpO2at the time point of starting to inject flubiprofen（T1）,10min（T2）,20min（T3） after inject it,before（T4）and after（T5）motion were monitoring continuously and recorded.Results the VAS scores of groupⅠand groupⅡwere different significantly.there was no significant difference between the two groups in the Ramsay scores.heart rate in groupⅡwere significantly higher than that of groupⅠat the time point of T3、T4、T5;systolic pressure and diastolic pressure in groupⅡwere significantly higher than that of groupⅠat the time point of T5;there was no difference between the two groups in SpO2.Conclusion flurbiprofen combined with sufentanil in PCIA can control postoperative motion pain of aged total hip replacement effectively.
