摘  要:背景单肺通气Cone lung ventilation,OLV)常用于胸科手术中,OLV不仅导致通气侧肺损伤,缺氧、低灌注状态也会导致萎陷侧肺出现严重的炎症反应,甚至造成远端器官的损伤,严重影响患者的预后及生存率。目的简要归纳总结OLV期间萎陷肺损伤可能的发生机制。内容OLV期间萎陷肺损伤可能的机制包括氧化应激反应、炎症反应、免疫反应、肺泡表面活性物质的减少及萎陷肺水肿。趋向通过探讨OLV萎陷肺损伤的机制为临床中预防和治疗OLV萎陷肺损伤提供理论依据。Background One-lung ventilation (OLV) is usually applied during thoracic surgery. But OLV could cause severe inflammatory reaction, even worse the acute lung injury (ALI) or acute respiratory distress syndrome (ARDS) which lead to really high mortality. Not only can the OLV harm the ventilated lung but also the hypoxia and the hypopeffusion could damage the collapsed lung. The injury of the collapsed lung may lead to severe inflammatory reaction even damage the distant organs, which will affect the prognosis and survival rate of the patients. Objective It is aimed to make a brief introduction of the possible mechanism which damaged the collapsed lung during OLV. Content The possible mechanism of the injury in the collapsed lung may focus on the following aspects: oxidative stress reaction, inflammatory reaction, decrease of the surface active substance and the collapsed lung edema. Trend This paper is for providing theory and evidence about how to prevent and treat the collapsed lung injury, by discussing its possible mechanism.
