摘　　要：目的了解娱乐场所女性服务人员(FEWs)艾滋病病毒(HIV)、梅毒螺旋体(TP)感染状况及其相关行为学动态变化特征,为进一步完善监测、干预措施及策略的制定提供参考。方法按云南省哨点监测方案,每年在固定时间、固定地点采用重复横断面调查方法收集人口学及相关行为学资料并采集血液标本,对血清学标本进行HIV、TP检测。结果 2005-2012年,共开展FEWs行为学监测7563人份,血清学监测7512人份。其中,艾滋病知识知晓率由2005年92.58%、2006年78.89%上升到2012年99.76%;最近一次安全套使用率均在95%以上,最近1个月每次安全套使用率由2005年的81.34%上升到2012年的94.83%;HIV抗体阳性检出率0.48%(1.55%～0.24%),呈下降趋势(χ2趋势=4.83,P<0.05);TP抗体阳性检出率1.85%(4.13%～1.18%),呈逐年下降趋势(χ2趋势=25.86,P<0.01);HIV、梅毒阳性检出率与最近1个月每次均用安全套率呈负相关(HIV:OR=0.932,95%CI:0.893～0.973;梅毒:OR=0.488,95%CI:0.431～0.552,P<0.05),与知识知晓率、最近一次用安全套率、吸毒者比例均未呈现一定的相互关系(P均>0.05)。结论玉溪市娱乐场所女性服务人员HIV流行仍处于较低水平,健康教育、推广使用安全套取得一定效果。Objective To understand the infection status of HIV and treponema pallidum （TP） and risk behaviors in female sex workers （FSWs） in Yuxi and provide evidence for the improvement of monitoring measures and intervention strategies. Methods According to the AIDS sentinel surveillance protocol of Yunnan province, cross-sectional survey was conducted among FSWs annually at fixed place and time to collect their demographic and behavioral data. Blood samples were take from them to detect HIV and TP. Results Totally 7563 FSWs were under behavior surveillance and 7512 were under serological surveillance from 2005 to 2012. The AIDS knowledge awareness rate increased from 92. 58% in 2005 and 78.89% in 2006 to 99.76% in 2012. The condom use rate at the latest sex was 〉 95%, the condom use rate in the last month increased from 81.34% in 2005 to 94. 83% in 2012. The overall HIV antibody positive rate was 0. 48 %, which declined from 1.55 % to 0. 24%,（X2tread = 4. 83, P 〈 0. 05 ）. The overall TP positive rate was 1.85%, which declined from 4. 13% to 1.18% （X2tread = 25.86, P 〈 0. 01 ）. HIV and TP positive rates were negatively correlated the last month condom use rate （HIV： OR = 0.932, 95% CI： 0.893 -0.973; syphilis： OR = 0. 488,95 % CI： 0. 431 -0. 552 ）, No correlations were observed between HIV and TP positive rates and the AIDS knowledge awareness rate, the latest condom using rate and drug user proportion. Conclusion HIV epidemic was still at low level among FSWs in Yuxi, indicating that health education and condom use promotion are effective in the prevention and control of AIDS and other sexual transmitted diseases.
