SLE、RA患者血清骨钙素水平

摘　　要：目的　骨钙素是骨形成的指标之一 ,为了解风湿性疾病患者骨代谢状况 ,我们对系统性红斑狼疮 ( SLE)、类风湿性关节炎 ( RA)两种自身免疫性疾病的血清骨钙素进行了测定。方法　用放射免疫分析 ( RIA)法测定了 4 0例 SL E、4 8例 RA患者及 30例正常对照者的血清骨钙素水平。结果　未经糖皮质激素治疗的 SLE患者血清骨钙素水平 ( 3.4 99± 1.793μg/ L )及服用糖皮质激素的 SL E患者血清骨钙素水平 ( 3.0 4 6± 1.0 35μg/ L )较正常对照组 ( 5.13± 1.96μg/ L)明显下降 ( P<0 .0 5,P<0 .0 5)。只有局部关节症状而未用糖皮质激素的 RA患者血清骨钙素水平 ( 5.319± 1.89μg/ L)与正常对照组比较无显著性差异 ( P>0 .1) ,伴有全身症状及服用糖皮质激素的 RA患者血清骨钙素水平较只有局部症状而未用糖皮质激素者下降 ( 5.319± 1.89μg/ L vs3.362± 1.0 13,P<0 .0 5)。结论　SL E患者存在骨形成障碍 ,骨转换率下降 ;伴有全身症状 (如发热等 )及服用糖皮质激素的Objective\ To understand the bone metabolism of rheumatism,we determined the serum osteocalcin level of patients with systemic lupus erythematosus (SLE) and rheumatoid arthritis (RA).Methods\ Forth patients with SLE,48 patients with RA and 30 matched normal controls were studied.The serum osteocalcin level was measured by RIA.Results The serum osteocalcin level of SLE patients on glucocorticoid therapy or not (3.499±1.793 μg/L,3.046±1.035μg/L,respectively)were significantly lower than that of the normal subjects( P <0.05,respectively).There was no significant difference between the RA patients with topical joint syndrome who did not receive glucocorticoid therapy(5.319±1.89μg/L)and the controls( P >0.05),but the serum osteocalcin level of the RA patients who had extraarticular syndrome or received glucocorticoids were lower than that of patients who had only topical joint syndrome and did not receive glucocorticoids( P <0.05). Conclusion Abnormal bone formation and decreased bone turnover exist in SLE.The serum osteocalcin level declines in RA patients with general syndrome and/or treated with glucocorticoids.
