摘  要:目的探讨腰椎脂肪酸变化及其与定量CT(quantitative computed tomography,QCT)所测骨密度的相关性。方法71例(30~80岁.平均56岁±15岁)女性受试者在磁共振腰椎常规扫描结束后采用单体素对L2~L4椎体进行波谱采集;此后采用QCT进行腰椎(1.2~L4)骨密度测量,并评估两者相关性。结果随年龄增大椎体饱和脂肪酸含量(fatfraction%,FF%)逐步增加,而椎体脂肪酸不饱和指数(unsaturation index,UI)呈现降低趋势,UI同QCT所测骨密度相关系数(r)为0.745(P〈0.01)。UI同FF%相关系数(r)为-0.813(P〈0.01)。结论随年龄增大椎体饱和脂肪酸含量逐步增加的同时伴有不饱和脂肪酸相对减少,并和椎体骨量存在相关性。磁共振波谱(magnetic resonanc espectroscopy,MRS)可在常规腰椎磁共振基础上无创性评估椎体骨髓脂肪酸变化状况.进而来评估其对于骨密度所带来的影响。Objective To investigate the correlation between the change of lumbar vertebral fatty acid and bone mineral density (BMD). Methods A total of 71 female volunteers (mean age 56-15 years, range 30--80 years) were recruited in this study. After routine MRI examination, single voxel (SV) was chosen for the l H MRS acquisition at L2--L4 vertebral body. All subjects underwent BMD measurement by quantitative computed tomography (QCT) at lumbar spine (L2--4). The correlation between fatty acid and BMD was evaluated. Results The fat fraction (FF% ) of vertebral saturated fatty acid ascended with aging. While unsaturaton index (UI) of vertebral body declined with aging and was strongly correlated with BMD (r=0. 745 P〈0.01). A significant association was observed between UI and FF% (r=-0. 813, P〈0.01). Conclusion There was a preferential increase in saturated lipids comparing to unsaturated lipids in vertebral bone marrow with aging. Vertebral MRS could be considered as a noninvasive examination in addition to routine spine MRI to demonstrate vertebral bone marrow changes, which could be helpful for further evaluation of vertebral BMD.
