摘 要:目的探讨慢性乙型肝炎、乙型肝炎肝硬化、肝细胞癌和慢加急性肝功能衰竭患者血清中半乳糖凝集素-9(Galectin-9)的水平及其与临床生物化学指标间的相关性。方法选择慢性乙型肝炎20例,乙型肝炎肝硬化16例,肝细胞癌14例,慢加急性肝功能衰竭19例和健康对照11例。ELISA法检测血清中Galectin-9水平的同时检测ALT、AST、TBil、HBVDNA和国际标准化比值(INR)。统计学处理采用t检验和方差分析。结果Galectin-9在慢加急性肝功能衰竭组为(2.743±1.711)ng/mL、肝细胞癌组为(2.453±1.517)ng/mL、慢性乙型肝炎组为(1.460±1.331)ng/mL、乙型肝炎肝硬化组为(1.168±0.764)ng/mL、健康对照组为(0.330±0.139)ng/mL,与健康对照组比较,差异均有统计学意义(f值分别为-4.63、4.61、-2.86和-3.58,均P〈0.05)。患者血清中TBil水平与Galectin-9有相关性,且有统计学意义(R2=0.0268,P=0.007),而ALT、AST、HBVDNA、INR均与Galectin-9无显著相关性。结论Galectin-9在慢加急性肝功能衰竭患者中水平偏高,与TBil有显著相关性,从而对预测这类患者疾病进展和预后有一定的临床意义。Objective To understand serum Galectin-9 expressions in patients with chronic hepatitis B (CHB), hepatitis B virus (HBV) related hepatic cirrhosis, hepatocellular carcinoma (HCC), and HBV-related acute-on-chronic liver failure (HBV-ACLF), and to explore its correlati6n with biochemical indexes of related diseases. Methods Twenty patients with CHB (male:13, female: 7), 16 with HBV related hepatic cirrhosis (male:ll, female:5), 14 with HCC (male:14, female:0), 19 with HBV-ACLF (male: 15, female:4), and 11 healthy controls (male:8, female:3) were enrolled. Serum Galectin-9 levels were detected by enzyme-linked immunosorbent assay (ELISA). Levels of alanine aminotransferase (ALT), aspartate aminotransferase (AST), total bilirubin (TBil), HBV DNA and international normalised ratio (INR) were also measured. Statistical analyses were performed by t test and analysis of variance. Results The levels of Galectin-9 were (2. 743±1. 711) ng/mL in HBV-ACLF group, (2. 453± 1. 517) ng/mL in HCC group, (1. 460± 1. 331) ng/mL in CHB group, (1. 168±0. 764) ng/mL in HBV related hepatic cirrhosis group, and (0. 330±0. 139) ng/mL in healthy controls. There were significant increased than those in healthy controls (t = -4.63, 4.61, -2.86, -3.58; all P〈0.05). Serum TBil levels were significantly associated withGalectin-9 (R2 = 0. 0268, P=0. 007), while the remaining indicators, such as ALT, AST, HBV DNA and INR, were not significantly correlated with the levels of Galectin-9. Conclusions Galectin-9 expression is significantly higher in patients with HBV-ACLF, and is correlated with TBil level, which can be regarded as an indicator of disease severity and prognosis of patients with HBV infection.
