摘  要:目的探讨局部应用重组人粒细胞巨噬细胞集落刺激因子(rhGM-CSF)对烧伤后残余创面转化生长因子β_1(TGF-β_1)、血管内皮生长因子(VEGF)表达的影响。方法 6例烧伤后残余创面患者,采用同体对照的研究方法,每例患者选取两处面积为16~25 cm^2烧伤后残余创面,随机分为试验组和对照组。试验组创面给予rhGM-CSF凝胶剂,对照组给予空白基质安慰剂,每日换药1次。于用药前及用药后7、14 d切取创面边缘组织标本,采用免疫组化、ELISA法检测TGF-β_1、VEGF的表达水平。结果用药前两组各项指标均无显著差异(P>0.05)。用药后7、14 d试验组TGF-β_1和VEGF积分光密度(ID值)分别为17 924±1 440、19 777±1 198和37 116±1 694、43 197±1 796,均高于对照组(15 653±1 258、17 199±1 780和21 706±1 339、23 322±1 555,P<0.01)。用药后7、14 d试验组TGF-β_1和VEGF含量分别为(2 588±218)、(2 734±202)pg·g^(-1)和(3 258±146)、(3 463±138)pg·g^(-1),均高于对照组[(2 429±136)、(2 562±100)pg·g^(-1)和(3 048±121)、(3 161±127)pg·g^(-1),P<0.05或P<0.01]。结论局部应用rhGM-CSF能促进创面TGF-β_1、VEGF的表达,有益于烧伤后残余创面的愈合。AIM To investigate the effects of recombinant human granulocyte- macrophage colony- stimulating factor (rhGM-CSF) on the expression of transforming growth factor-β1 (TGF-β1) and vascular endothelial growth factor (VEGF) expression in residual burn wounds. METHODS The study was a randomized and self-controlled clinical trial. Six patients with residual burn wounds were enrolled. Two residual burn wounds with area of 16 - 25 cm2 were collected from each patient and were randomly divided into group A (with treatment of rhGM-CSF gel) and group C (with treatment of placebo). The wound dressing was changed daily. The skin tissue samples around the wounds before and at the 7 th and 14 th days after the treatment were harvested for evaluation of expression of TGF-β1 and VEGF by immunohistochemistry and ELISA. RESULTS No significant differences in the expression of TGF-β1 and VEGF were observed before the treatment (P 〉 0.05). The values of integral optical density (ID) of TGF-β1 and VEGF examined by immunohisto-chemistry were significantly increased in the group A compared to those in the group C at both the 7 th and 14 th days after the treatment, respectively (TGF-β1, 17 924 ± 1 440 vs. 15 653 ±1 258 at the 7 th day and 19 777± 1 198 vs. 17 199± 1 780 at the 14thday; VEGF, 37 116± 1 694 vs. 21 706± 1 339 at the 7 th day and 43 197 ± 1 796 vs. 23 322 ± 1 555 at the 14 th day, all reached the statistic difference P 〈 0.01 ). Similar up- regulation of TGF- β1 and VEGF expression in group A was observed by ELISA at the 7 th and 14 th days after the treatment (TGF-β1, (2 588± 218) pg·g^-1 vs. (2 429 ± 136) pg.g-i at the 7 th day, P 〈 0.05 and (2 734± 202) pg·g^-1 vs. (2 562 ± 100) pg·g^-1 at the 14 th day, P〈 0.05; VEGF, (3 258 ± 146) pg·g^-1X vs. (3 048 ± 121) pg.g-i at the 7 th day, P〈 0.01 and (3 463± 138) pg·g^-1 vs. (3 161 ± 127) pg·g^-1 at the 14 th day, P 〈 0.01).
