摘 要:目的探讨64层螺旋CT在复杂型先天性心血管疾病中的诊断价值。资料与方法回顾性分析经手术证实的59例复杂型先天性心血管疾病患者的CT资料,将复杂型先天性心血管病分为心脏畸形、心脏血管连接部及心脏周围血管畸形,并将诊断结果与手术所见进行对照,采用多平面重组、最大密度投影和曲面重组等对图像进行三维重建。结果 59例患者共198处病变,其中心脏畸形129处,CT诊断正确114处,漏诊15处,诊断准确率为88.37%(114/129);心脏血管连接部及心脏周围血管畸形69处,CT诊断正确67处,漏诊2处,诊断准确率为97.10%(67/69)。结论复杂型先天性心血管疾病解剖变异较大,尤其是心脏血管连接部及心脏周围血管畸形,64层螺旋CT可以作为一种有效地评价复杂型先天性心血管病的初选检查手段。Purpose To discuss the diagnostic value of 64-slice CT in complex congenital Heart disease. Materials and Methods The CT data of 59 patients with complex congenital heart diseases verified by surgery were retrospectively studied. All data were divided into two groups (the group of intracardiac deformity and the group of extra- and peripheral cardiac vascular deformity), and the diagnostic results were further compared with those of surgery. Reconstructions were pertbrmed by means of multi-planar reconstruction (MPR), maximum intensity projection (MIP) and curved planar reformation (CPR). Results A total of 198 cardiac detbrmities were tbund in 59 patients, among which there were 129 intracardiac deformities [114 diagnosed by CT, 15 missed, accuracy rate 88.37%, (114/129)] and 69 extra- and peripheral cardiac vascular de|'ormities [67 diagnosed, 2 missed, accuracy rate 97.10% (67/69)]. Conclusion 64-stice CT can be used as a primary method to check complex congenital heart diseases usually associated with fairly dramatic anatomic detbrmity, especially those with extra- and peripheral cardiac vascular det'ormity.
