摘　　要：目的采用速度向量成像技术(VVI)和动脉健康评估(AHP)软件评价多囊卵巢综合征(PCOS)患者的颈动脉弹性。方法收集35例PCOS患者(PCOS组)和30名年龄匹配的健康女性(对照组),测量身高、体质量、腰围、臀围、血压,计算体质量指数和腰臀比,测定血糖、血脂和胰岛素,计算胰岛素抵抗指数(IRI);行右侧颈动脉超声检查,采用VVI测量应变、应变率及各参数达峰时间,AHP测量内中膜厚度(IMT)、弹性系数和扩张性系数。结果 PCOS组体质量指数和腰臀比明显高于对照组(P均<0.05);PCOS组空腹血糖、甘油三酯、血浆总胆固醇、低密度脂蛋白与对照组相比差异无统计学意义,但空腹胰岛素和IRI明显高于对照组(P均<0.0001);PCOS组应变小于对照组(P<0.05),其余VVI参数与对照组相比差异无统计学意义(P均>0.05);PCOS组弹性系数与扩张性系数低于对照组(P均<0.05),IMT与对照组相比差异无统计学意义(P均>0.05)。结论 VVI技术和AHP软件可用于评价PCOS患者动脉硬化早期改变,早期发现其心血管病危险因素。Objective To evaluate the elasticity of carotid artery in polycystic ovarian syndrome （PCOS） patients using velocity vector imaging （VVI） and artery health package （AHP）. Methods The height, weight, waist, hipline and blood pressure of 35 PCOS patients （PCOS group） and 30 normal women （control group） were recorded, and weight index and waist-to-hip ratio were calculated. Then blood samples, such as insulin, glycimia, trighycerides were tested, and insulin resistance index （IRI） was calculated. All subjects underwent carotid artery ultrasonography. VVI was used to measure the max velocity （Vmax）, strain （S） and strain rate （SR）. AHP was used to measure intima media thickness, distensibility （D） and elasticity （E）. Results The weight index and weight-to-hip ratio in PCOS group were higher than those in control group （both P〈0. 05）. Insulin and IRI in PCOS group were higher than those in control group （both P〈0. 0001）, but other biochemical parameters were not statistically different between the two groups. Strain, D and E in PCOS group were lower than those in control group （all P〈0.05）, while other parameters of ultrasonography were not statistically different between the two groups （all P〈0. 05）. Conclusion VVI and AHP can be used to detect early decreasing of elasticity of carotid in PCOS patients, indicating increasing of cardiovascular risk.
