摘　　要：目的:探讨以MRI诊断的不同程度早产儿脑室周围-脑室内出血(PV-IVH)的相关危险因素。方法:以MRI(T1WI-T2WI)和弥散加权成像诊断PV-IVH,并根据损伤程度进行分级。详细统计临床资料,进行单因素分析,对单因素分析有统计学意义的因素行PV-IVH的Logistic回归分析。回顾性分析天津医科大学第二医院2008年7月-2012年12月的PV-IVH患者相关危险因素。根据PV-IVH分级分为轻度和重度PV-IVH,并进行有序多分类Logistic回归分析。结果:男性、阴道分娩、产前母亲感染、高碳酸血症、低碳酸血症是早产儿PV-IVH的危险因素;男性、阴道分娩、双胎、高碳酸血症及循环功能障碍更易造成重度PV-IVH。结论:理解PV-IVH的危险因素有助于探索阻止发生早产儿神经系统异常的有效策略,围产期护理对提高早产儿质量极其重要。Objective： To explore the corretated risk factors for periventricular - intraventricular hemorrhage in preterm infants and the difference of clinical manifestation among varying degree of PV - IVH. Methods： Conventional MRI and diffusion - weighted imaging （DWI） were performed in 684 preterm infants. A retrospective study from preterm infants was conducted and admitted to the neonatal inten- sive care unit of the Second Hospital of Tianjin Medical University between July 2008 and December 2012. At the multivariate Logistic regres- sion model, Logistic regression analysis was used to evaluate the independent relationship among variables found to be significantly different between the two groups. According to the classification, PVL was divided into mild, major PV - IVH, and the ordinal logistic regression a- nalysis was used. Results： At the multivariate Logistic regression model： male, vaginal delivery, prenatal infection, hypercapnia, hypocap- nia were associated with an increased risk of PV - IVH. Males, vaginal delivery, twin, hypercapnia and circulatory dysfunction leaded to ma- jor PV - IVHConcluslon： Identifying the risk factors has the potential to allow for the development of effective strategies for prevention of many neurodevelopmental problems of premature infants. The data emphasize the importance of obstetric and perinatal care to improve out- comes of premature infants.
