曲美他嗪对老年慢性心力衰竭自主神经功能的影响观察

摘　　要：目的探讨曲美他嗪对老年慢性心力衰竭患者自主神经功能的影响。方法将解放军第97医院于2011年1月至2011年6月收治的68例老年慢性心力衰竭患者按照随机数字表法分为观察组和对照组,各34例,所有患者采用心力衰竭常规药物治疗,观察组加用曲美他嗪20 mg,每日3次,口服,分别于用药前及用药12周后检查患者的心脏功能与心率变异性(HRV)指标。结果两组患者治疗后均能明显改善慢性心力衰竭患者的心脏射血分数(P<0.05),且观察组改善情况优于对照组(P<0.05);两组患者治疗后HRV各指标均升高(P<0.05),且观察组与对照组比较,差异有统计学意义(P<0.05)。结论除常规治疗外,应用曲美他嗪治疗老年慢性心力衰竭能显著提高患者的HRV,改善自主神经功能,具有不依赖于其他改善血流动力学药物的显著正性作用,改善心肌细胞代谢可能是此方面作用的主要基础。Objective To investigate the effects of trimetazidine on the autonomic nervous function in elderly patients with chronic heart failure.Methods Sixty-eight elderly patients with heart failure admitted by Chinese PLA 97th Hospital from January 2011 to June 2011 were randomly divided into observation group and control group.All patients were treated with conventional drug therapy,while observation group was treated with additional 20 mg trimetazidine three times / day orally.Echocardiography and 24-hour heart rate variability were tested before treatment and 12 weeks after the treatment.Results The HRV parameters were significantly increased after treatment in the two groups（P 0.05）,and the observation group was increased more significantly（P 0.05）.Ejection fraction of the patients in both groups increasd（P 0.05）,andin the observation group was more significantly increased than the control group（P 0.05）.Conclusion Besides the routine treatment,trimetazidine may play some important and remarkably positive roles in the improvement of HRV and autonomic nervous function in elderly patients with chronic heart failure.Trimetazidine is not dependent on other haemodynamic changing drugs,and improving energy metabolism of myocardial cells might be the main basis of the function.
