蚕食清创法联合立奇膏外敷治疗糖尿病足的临床研究

摘　　要：目的观察运用蚕食清创法加自制立奇膏外敷治疗3～4级糖尿病足的临床疗效,以探索一种治疗效果显著、治愈时间短、价格低廉的治疗方法。方法方便性抽样选择住院及门急诊换药的3～4级糖尿病足溃疡患者36例,运用蚕食清创法加上海市第二人民医院立奇膏外敷进行伤口处理,观察治疗前后伤口面积、血糖、医疗费用的变化以及总有效率。结果 36例患者中愈合33例,总有效率为91.7%;平均换药费用与入院前相比有明显降低;伤口面积、血糖值与治疗前比较,差异均有统计学意义(均P<0.05)。结论运用蚕食清创法加立奇膏外敷治疗3～4级糖尿病足效果显著,治愈时间短,价格低廉,值得临床推广应用。Objective To observe the clinical efficacy for use of eroded debridement combined with Li Qi Ointment in patients with 3-4 grade diabetic foot,in order to explore a treating method that has significant effect ,shorten curing time and lower cost. Methods A total of 36 patients with 3-4 grade diabetic foot admitted to our inpatient and outpatient department were enrolled randomly, and were treated with eroded debridement combined with Li Qi ointment self-made by our hospital. The change of wound area, blood sugarand medical expense of wound treatment before and after the treatment were compared and the total efficiency were observed. Results In 36 cases,amount of 33 cases were cured with the effective rate 91.7%. Compared with the medical dressing expense before admission ,the average dressing expense was significantly reduced after treatment. There were significant differences of wound area and blood sugar between pretherapy and post-treatment （P〈0.05）. Conclusion The eroded debridement combined with Li Qi ointment is effective in treating 3-4 grade diabetic foot with significant efficient , shortening treatment time and lowering medical expense, which is worthy for clinical popularization and application.
