活血清热法对骨科多发伤患者血清TNF-α、IL-1、IL-6因子水平的调控

摘　　要：目的探讨活血清热法中药对骨科多发伤TNF-α、IL-1、IL-6因子水平变化的影响。方法观察骨科多发伤患者63例,随机分为活血清热法中药组和常规对照组进行治疗,比较两组患者全身炎症反应综合征(SIRS)发生率以及TNF-α、IL-1、IL-6因子水平变化。结果伤后当天患者各炎症因子均明显升高,伤后3d达到峰值,伤后7d中药组较前明显下降,对照组则未见明显下降,两组间比较差异有显著性意义(P<0.05)。中药组SIRS发生率为3.13%,少于对照组25.80%,差异有显著意义(P<0.05)。结论骨科多发伤患者炎性反应与预后相关,运用活血清热法可有效的降低创伤后炎症因子水平,预防SIRS发生。Objective To discuss the influence of HuoXueQingRe decoction for regulating TNF-α, IL-1 and IL-6 levels after orthopedics multiple injury. Method All 63 patients who suffered muhiple trauma were randomly divided into two groups, treated group（32 cases were given administration with HuoXueQingRe decoction） and controlled group （31 cases were given administration with routine treatment）. Clinical preventive effects and inflammatory cytokines levels were compared in the two groups. Results Analysised from preventive effect of the two groups, the SIRS incidence rate of treated group was 3.13% , and the number above was respectively 25.8% in controlled group. All patients＇inflammatory cytokines levels increased significantly the first day after trauma,and the levels increased highest in the third day. After treatment, the increased inflammatory cytokines levels of treated group decreased remarkably, while those of controlled group hardly reduced, with significant difference between the two groups. Conclusion The inflammatory cytokines levels are correlated positively with prognosis after major orthopedics multiple trauma. HuoXueQingRe decoction could effectively reduce these inflammatory cytokines levels after major orthopedics multiple injury.
