早期良肢位摆放对脑卒中病人瘫痪肢体功能康复的效果观察

摘　　要：[目的]评价早期良肢位摆放在脑卒中病人偏瘫肢体功能康复中的效果。[方法]选取200例脑卒中病人为研究对象,随机分为观察组和对照组,每组100例。两组病人均给予神经内科常规治疗及护理,于生命体征平稳、神经学症状不再发展后48h,开始进行患侧肢体的被动运动,从少到多,从粗到细,由近及远,逐步过渡到主动运动。观察组在此基础上在病人入院当天即给予良肢位摆放,并贯穿康复全过程。分别于干预前、干预后半个月、1个月、3个月、6个月进行Fugl-Mey-er运动功能评分,评价病人肢体功能恢复情况和并发症情况。[结果]观察组病人Fugl-Meyer运动功能评分明显高于对照组(P<0.01),肩关节疼痛、足下垂、足内翻、上肢屈肌痉挛、下肢伸肌痉挛等并发症发生率也较对照组明显下降(P<0.05)。[结论]对脑卒中病人实施早期良肢位摆放,能有效降低残障程度,减少并发症发生,提高病人的生活质量。Objective. To evaluate the effect of early limbs in good position placed for functional rehabilitation of paralyzed limbs in stroke patients. Methods：A total of 200 cases of stroke patients were selected as objects of study and were divided into observation group and control group, 100 cases in each. Two groups of patients were given routine neurologic treatment and care. The patients when in 48h when vital signs were stable and after neurologic symptoms developed no longer, ipsilateral limb passive move- ment began, from less to more, from coarse to fine, from near and far, a gradual transition to active exercise. Based on this, observation group pa- tients received placing limbs in good position on the same day when the pa- tients were admitted, and throughout the whole process of rehabilitation. Fugl - Meyer motor scored to evaluate the recovery of limb function and complications respectively before the intervention and in half a month,one month, three months, six months after intervention. Results- Fugl - Meyer motor function score of observation group of patients was significantly higher （P^0.01）, incidence of shoulder pain, foot drop, foot varus, upper limb flexor spasms, leg extensor spasms and other complications was de- creased significantly compared with the control group （P%0.05）. Conclu- sion：The implementation early limbs in good position placed for stroke pa- tients can effectively reduce the degree of disability of stroke patients, re- duce complications and improve their quality of life.
