组合型人工肾治疗血液透析患者皮肤瘙痒的临床研究

摘　　要：目的对组合型人工肾治疗维持性血液透析患者皮肤瘙痒症进行临床研究。方法将54例维持性血液透析患者,随机分为组合型人工肾组、血液透析滤过组及血液透析组3组。观察并记录首次透析前后及透析第8、16周后患者皮肤瘙痒的临床症状评分、疗效情况,对各组指标进行统计分析。结果组合型人工肾组首次透析后及透析8、16周后与首次透析前相比,患者的皮肤瘙痒临床症状评分显著下降(P<0.05);血液透析滤过组首次透析前后皮肤瘙痒临床症状评分变化差异无统计学意义(P>0.05),透析8、16周后评分明显下降(P<0.05);血液透析组首次透析前后及透析8、16周后皮肤瘙痒临床症状评分变化差异无统计学意义(P>0.05)。16周后,临床有效率:组合型人工肾组最高达89%,血液透析滤过组56%,血液透析组11%。结论组合型人工肾能有效地缓解皮肤瘙痒症状,其效果优于血液透析滤过及血液透析,而血液透析组疗效不佳。故组合型人工肾治疗能有效地改善尿毒症患者的皮肤瘙痒,值得推广应用。Objective To carry on clinical study combined hemodialysis-hemoperfusionin in treating the maintenance hemodialysis patients with cutaneous pruritus. Methods Fifty-four maintenance hemodialysis patients were randomly, divided into three groups： combined hemodialysis-hemoperfusion （HD＋HP） group, hemoperfusion （HDF） group, hemodialysis （HD） group. The clinical symptom and efficacy of the patients＇ cutaneous pruritus were recorded before and after the first hemodialysis, 8, 16 weeks after hemodialysis, and then analyzed, scored these statistics. Results For HD＋HP group, compared with pre first hemodialysis, the symptom score of cutaneous pruritus decreased significantly both post first hemodialysis and 8, 16 weeks after first hemodialysis （P〈0.05）; for HDF group, the symptom score of cutaneous pruritus didn＇t change significantly post first hemodialysis （P〉0.05）, while decreased significtmtly 8, 16 weeks after first hemodialysis （P〈0.05）; for HD group, the symptom score of cutaneous pruritus didn＇t change signifcantly both post first hemodialysis and 8, 16 weeks after first hemodialysis （P〉0.05）. Conclusion HD＋HP can effectively alleviate the symptoms of cutaneous pruritus, better than the HDF and HD, the HD is poor efficacy. So combined hemodialysishemoperfusion can effectively improve cutaneous pruritus of patients with uremia, should be widely applied.
