持续血液净化治疗多器官功能障碍综合征的临床观察

摘　　要：目的探讨持续血液净化在多器官功能障碍综合征(MODS)治疗中的临床疗效。方法回顾性分析深圳市南山区人民医院自2011年1月至2012年5月间收治的MODS患者84例,其中43例采用持续血液净化疗法(净化组),41例患者采用传统疗法(传统组),对两组的疗效进行比较。结果两组治疗结束后,净化组BUN、SCr、K+、pH、PaO2分别为(12.98±3.89)mmol/L、(231.26±78.65)μmol/L、(4.25±0.26)mmol/L、7.26±0.04、(83.54±5.83)mm Hg,传统组分别为(18.46±3.59)mmol/L、(426.31±54.59)μmol/L、(5.32±0.86)mmol/L、7.34±0.09、(53.64±5.12)mm Hg,净化组显著优于对照组,差异有统计学意义(P<0.05)。净化组和传统组的总有效率分别为76.74%和53.66%,差异有统计学意义(P<0.05)。结论持续血液净化治疗MODS,可有效改善患者生命体征及血气,生化指标,缓解临床症状,减轻肝肾及其他脏器损伤,提高治疗有效率。Objective To investigate the clinical application effect of continuous blood purification in the treatment of multiple organ dysfunction syndrome （ MODS ）. It provides the reference for clinical. Methods From January 2011 to May 2012,84 MODS cases from People＇s Hospital of Nanshan District were retrospective analyzed,including 43 patients treated with continuous blood purification therapy （purification group ） ,41 patients treated with traditional therapy（ traditional group） ,the effect of the two groups were compared. Results after the treatment, BUN, SCr, K ＋ , pH, PaO2 of purification group were （12.98±3.89） mmol/L、（231.26±78.65） μmol/L、（4.25±0.26） mmol/L、7.26±0.04、（83.54±5.83） mm Hg mm Hg respectively, and the traditional group were （18.46±3.59） mmol/L、（426.31±54.59） μmol/L、（5.32±0.86） mmol/L、7.34±0.09、（53.64±5.12） mm Hg respectively, purification group was better than control group with statistically significant difference（ P 〈 0.05 ）. After treatment, total effectiveness of purification group was 76.74% ,of the traditional group was 53.66% , the difference was statistically significant （ P 〈 0.05 ）. Conclusion Continuous blood purification treatment for MODS can effectively improve the total effective rate, improve the life signs and blood and gas, biochemical indexes, relieve the clinical symptoms, lighten the injuries of liver, kidney and other organs.
