负压封闭引流技术联合游离植皮治疗深度烧伤感染创面的临床研究

摘　　要：目的探讨负压封闭引流技术(VSD)联合游离植皮治疗深度烧伤感染创面的临床效果。方法选取2010年1月至2013年1月间铜川市矿务局中心医院收入的深度烧伤感染创面患者60例为研究对象,采用随机数字表法分成2组,每组30例,对照组予以植皮加压包扎治疗,观察组则予VSD联合游离植皮治疗,比较两组治疗后的优良率、0~15 d清洁率及术后发热、抗生素使用、住院天数等临床指标。结果对照组良好率为73.3%(22/30),0~15 d清洁率为26.7%(8/30);观察组良好率为93.3%(28/30),0~15 d清洁率为53.3%(16/30),上述两指标组间比较差异显著(均P<0.05);对照组与观察组在发热、抗生素使用、住院、终末手术、疼痛各项的所用时间分别为[(5.5±1.3)d vs(2.1±0.6)d]、[(20.3±9.4)d vs(7.9±5.2)d]、[(8.3±12.5)d vs(25.3±8.4)d]、[(27.4±11.4)d vs(12.4±7.3)d]、[(9.7±3.6)d vs(3.6±2.7)d],观察组除住院时间多于对照组,其他各项均显著少于对照组所用时间,差异均有统计学意义(P<0.05);另外两组在毛囊炎、出血、再次感染、张力性水泡等并发症方面比较也有明显差异性(P<0.05),以上均具有统计学意义。结论 VSD联合游离植皮治疗深度烧伤感染创面临床效果满意,其操作简单、护理方便。Objective To investigate the clinical effects of vacuum sealing drainage VSD） combined with skin graft treatment for deep bum wound infection. Methods From January 2010 to January 2013,60 patients with deep burn wound infection from Tongchuan Mining Bureau Central Hospital were selected for the study, and divided into two groups of 30 patients in each：the control group received skin grafts pressure dressing therapy, while the observation group received VSD combined skin graft treatment, the good rate, 0-15 d cleaning rate and postoperative fever, antibiotic use,length of hospital stay and other clinical indica- tors of the two groups after treatment were compared. Results In the control group, the incidence of good outcomewas 73. 3% （22/30） ,0-15 d cleaning was 26.7% （8/30） ;in the observation group,the corre- sponding data was 93.3% （ 28/30 ） and 53.3% （ 16/30 ） respectively, there were significant differences between the two groups （ all P 〈 0.05 ） ; fever, antibiotic use, hospitalization, terminal surgery, pain time of the control group and observation group of the were respectively： [（5.5±1.3）d vs （2.1±0.6）d]、[（20.3±9.4）d vs （7.9±5.2）d]、[（8.3±12.5）d vs （25.3±8.4）d]、[（27.4±11.4）d vs （12.4±7.3）d]、[（9.7±3.6）d vs （3.6±2.7）d] , except the hospitalization time, other indexes of the observation group were all significantly less than the control group （ P 〈 0.05 ） ; besides, folliculitis, bleeding, re-infection, and other complications of tension blisters of the two groups were all statistically si^aifieantly different （ P 〈 0.05 ）. Conclusion VSD combined with skin graft treatment for deep bum wound infection has satisfactory clinical results,with the features of simple operation and easy care.
