地佐辛用于单侧下肢静脉曲张高位结扎加硬化剂注射术的临床观察

摘　　要：目的探讨地佐辛用于单侧下肢静脉曲张高位结扎加硬化剂注射术的效果。方法选择60例ASAⅠ～Ⅱ级拟行单侧下肢静脉曲张高位结扎加硬化剂注射术的患者，采用计算机随机数字法分为F组和D组各30例。F组静脉注射芬太尼2μg／kg；D组静脉注射地佐辛0．1mg／kg。观察两组患者给药前（T1）、手术开始（T2）、导管置入（T3）、术毕（T4）的MAP、HR、RR、SpO2。记录术中、术后2、6及24小时疼痛评分（VAS）。观察围术期恶心呕吐、头昏、呼吸抑制等不良反应发生情况。结果两组患者T2时点MAP、HR、Sp02、RR均明显低于T1时点（P〈0．05）；D组T2时点Sp02、RR明显高于F组（P〈0．05）。VAS疼痛评分两组差异无统计学意义（P〉0．05）。D组恶心呕吐、头昏、呼吸抑制发生率明显低于F组（P〈0．05）。结论地佐辛用于单侧下肢静脉曲张高位结扎加硬化剂注射术，其镇痛效果能达到临床麻醉需要，麻醉期间不良反应少，值得临床推广应用。Objective To evaluate the effect and safety of dezocine in high ligation and injection of sclerosing foam for vari- cose veins of unilateral lower extremity. Methods 60 patients ASA Ⅰ-Ⅱ , scheduled for high ligation and injection of sclerosing foam for varicose veins of unilateral lower extremity were randomly divided into two groups （ The F group and the D group）. The patients in the F group were receiving fentanyl 2 ug/kg and patients in theD group were received dezocine 0. 1 mg/kg. Results The MAP, HR, RR,SpO2 at these four times ：before anesthesia（T1 ） ,the beginning of the surgery（T2 ） ,the time of placing the cather（T3 ）and the end of the surgery（ T3 ） were recorded. The vas scores during the surgery ,2 hours,four hours and 6 hours after the surgery were recorded. The side effect such as nausea and vomiting and respiratory depression were recorded during the perioperative. The MAP and HR at the be- ginning of the surgery were significantly lower in both Groups at the times of T1 （ P 〈 0.05 ）. The SpO2 and RR in the D group were higher than the F group. There were no significant differences of the VAS scores between the two groups. The side effects of nausea and vomiting and respiratory depression in the D group were much lower than these in the F group（P 〈 0. 05）. Conclusion It is safe and effective for dezocine in high hgation and injection of sclerosing foam for varicose veins of unilateral lower extremity.
