摘  要:目的探讨臂踝脉搏波传导速度(brachial-ankle pulse wave velocity,baPWV)、踝臂指数(ankle-brachial index,ABI)和高敏感C反应蛋白(hsCRP)对动脉粥样硬化程度的诊断意义。方法随机抽取高血压前期和高血压患者各50例作为研究对象,并选择50例血压正常的健康体检者作为对照。采用多普勒彩色超声检测颈动脉内膜中层厚度(intima-media thickness,IMT)和斑块形成情况,测定baPWV、ABI和hsCRP,比较不同血压组baPWV、ABI、hsCRP和IMT的变化情况和相关性。结果 ABI、hsCRP、IMT增厚及斑块发生在高血压前期组和对照组之间差异均无统计学意义(P>0.05)。高血压组的ABI、baPWV、IMT增厚及斑块发生显著高于高血压前期组和对照组(P<0.05),hsCRP显著降低(P<0.05)。baPWV与hsCRP和IMT呈正相关关系(r分别为0.51和0.27,P均<0.05)。结论 baPWV可作为评估高血压前期动脉硬度有效且简便易行的指标。ABI和hsCRP浓度的变化在高血压组明显增高,对于动脉粥样硬化具有一定的预测价值。Objective To explore the diagnostic significance of brachial-ankle pulse wave velocity (baPWV), ankle brachial index (ABI) and high sensitive C-reactive protein (hsCRP) in the extent of atherosclerosis. Methods Fifty patients with pre-hyperten- sion and 50 patients with hypertension were randomly selected for this study. At the same time ,50 subjects with normal blood pressure were used as controls. BaPWV, ABI, hsCRP, and carotid arteries intima media thickness (IMT)and plaque were compared among the groups. Results No significant differences in ABI, hsCRP and IMT were found between the pre-hypertension and control groups. However, ABI, baPWV and IMT in the hypertension group were significantly higher while hsCRP was significantly lower than that in the pre-hypertension and control groups ( all P 〈 0.05 ). BaPWV was positively related to hsCRP and IMT (r = 0. 51 and 0. 271, respective- ly;all P 〈 0. 05). Conclusion BaPWV can be used as a simple and effective indicator for arterial stiffness in pre-hypertension pa- tients. ABI and hsCRP may have predictive value for atherosclerosis.
