摘  要:目的观察脉冲噪声致纯音听力损失前畸变产物耳声发射(DPOAE)异常的自然转归及硫辛酸对其转归的影响。方法纯音听阈正常、DPOAE异常者26例(41耳),随机分为对照组和治疗组,每组各13例。治疗组给予硫辛酸600mg/日静滴治疗,对照组给予安慰剂治疗,治疗后第2、4、6、8、10天,分别进行纯音听阈及DPOAE检查。结果治疗前两组各对应频率的纯音听阈及DPOAE幅值均无显著差别,DPOAE异常频率主要发生在3、4、6、8kHz。经过治疗后,两组的纯音听阈无明显变化,对照组异常频率的DPOAE的幅值自第8天开始,显著大于治疗前。治疗组异常频率的幅值自第4天开始显著大于治疗前。从治疗后第4天,治疗组异常频率的幅值显著高于对照组对应时间和频率的幅值。结论人耳对脉冲噪声致纯音听力损失前DPOAE幅值异常有一定的自行修复能力,硫辛酸对其恢复有明显的促进作用,提示硫辛酸对爆震性聋有预防作用。Objective To investigate the effect of lipoic acid on abnormally low distortion product otoacoustic emissions (DPOAE) with no hearing loss following military noise exposure. Methods Twenty six (41ears) artillerymen,with normal pure tone hearing thresholds and abnormal DPOAEs amplitudes following military noise exposure were randomly divided into 2 groups. The test group with 13 young adults (21 ears) received intravenous drip of lipoic acid (600 mg/day). The control group with 13 young adults (20 ears) received intravenous drip of placebo. Pure tone audiomety and otoacoustic emission were per- fomied on Days 2, 4, 6, 8 and 10 after treatment. Pure tone hearing thresholds and amplitudes of DPOAEs were compared and analyzed. Results Statistical analysis showed that prior to the treatment the amplitudes of DPOAE at 3, 4,6 and 8 kHz did not differ significantly between the two groups. DPOAE amplitudes in the control group increased significantly on the 8th day after treatment. The amplitudes in the test group increased significantly on the 4th day after treatment, with a significant differ- ence between the test and control groups. Conclusion Lipoic acid seems to significantly improve DPOAE amplitudes toward normal in individuals with subclinical auditory system impairment caused by military noise exposure.
