5-氨基酮戊酸光动力疗法联合卡介菌多糖核酸治疗尖锐湿疣31例疗效观察

摘　　要：目的观察5-氨基酮戊酸光动力疗法(ALA-PDT)联合卡介菌多糖核酸治疗尖锐湿疣的临床疗效。方法将入选的61例患者随机分为两组,治疗组31例,用ALA-PDT治疗联合卡介菌多糖核酸2mL肌肉注射,隔日1次;对照组30例,仅用ALA-PDT进行治疗,1次/周,不超过4次,皮损完全消退后停止治疗。结果治疗组痊愈率93.50%(29/31),有效率100%(31/31),对照组痊愈率90.00%(27/30),有效率96.70%(29/30),两组痊愈率差异无统计学意义(P>0.05)。治疗组复发率6.50%(2/31),对照组复发率26.70%(8/30),两组患者的复发率差异有统计学意义(P=0.033)。两组患者治疗后均无明显不良反应。患者皮损形态呈扁平丘疹状者对治疗敏感性低。结论 ALA-PDT联合卡介菌多糖核酸治疗尖锐湿疣较单独ALA-PDT治疗的疗效好,复发率低。Objective To observe the effect of 5-aminolevulinic acid photodynamic therapy（ALA-PDT） combined with polysaceharide nucleic acid fraction of bacillus calmette-guerin （BCG-PSN） in the treatment of condyloma aeuminatum. Methods Sixty-one cases who suffered from condyloma acuminatum were divided into two groups randomly, the lesions were like cauliflower shape, or cockscomb shape, others were like flat papular or carpet, The treatment group will be treated by ALA-PDT combined with BCG-PSN（ intramuscular injection of 2ml once every other day） while the control group be treated only by ALA-PDT. The ALA-PDT treatment was repeated weekly if necessary, but no more than 4 times. Local adverse Reactions, extent of resolution of the lesions and recurrence rate were evaluated after treatment. Results The recovery rate of the treatment group was 93.50% （29/31） and 90.00% （27/30） were founded in the control group. There were no significant difference in the two groups（ P 〉 0.05 ） ; The recurrence rate of the treatment group was 6.50% （2/31 ） and the control group was 26.70% （8/30） , the two groups had significant differences （P = 0. 033 ）. There was no adverse event in either group. Patients with lesions like the flat papular has a low sensitivity to treatment. Conclusion Photodynamic therapy combined with BCG-PSN for the treatment of condyloma acuminatum was advantaged and with low recurrence rate than applied only photodynamie therapy method.
